摘 要:手术创伤带来的剧烈疼痛与应激反应显著影响预后及就医体验,麻醉手术期间如何准确评估疼痛应激反应程度,是麻醉疼痛管理的关键。灌注指数(Perfusion index,PI)作为一项反映脉动血流的监测项目,能反映监测部位的血流灌注情况。创伤刺激引发机体的一系列生理调节反应,使体内血流灌注的分布发生改变,末梢部位血流灌注减少,这一调节过程可通过反映灌注的PI得到体现。现有研究明确了PI与疼痛应激之间的相关性,且具有可靠、客观、量化等优势,为临床监测患者的疼痛程度以及应激水平提供了新思路。Severe pain and stress reaction caused by surgical trauma significantly affect the prognosis and medical experience.How to accurately evaluate the degree of pain stress reaction during anesthesia surgery is the key to anesthesia pain management.As a monitoring item reflecting pulsatile blood flow,perfusion index(PI)can reflect the blood flow perfusion at the monitoring site.Trauma stimulation triggers a series of physiological regulatory responses of the body,which changes the distribution of blood flow perfusion in the body and reduces the blood flow perfusion in the peripheral part.This regulatory process can be reflected by the PI index reflecting the perfusion.The existing research has clarified the correlation between PI and pain stress,and PI has the advantages of reliability,objectivity and quantification,which provides a new idea for clinical monitoring of patients'pain degree and stress level.
