摘　　要：支气管扩张症作为第三大慢性气道疾病,其全程管理却常被忽视,这可能加剧急性加重和肺功能下降情况,带来沉重的疾病负担。本文对支气管扩张症防治现状、中医理论认识、中医药分期及分表型治疗支气管扩张症的进展进行综述。认为在不同病程阶段、不同临床表型中,中西医结合、中西药并用可更好地改善症状、延缓肺功能下降、促进肺部病灶吸收和减少急性加重,充分发挥中医药治未病、辨证施治、多靶点干预的独特优势,全程参与病情管理,将有助于构建慢性肺病中西医联合治疗方案。应积极探索中医药对不同支气管扩张症表型的疗效特点,尤其是难治性支气管扩张症(继发微生物定植、反复急性加重表型、伴反复咯血等),积累临床和研究数据,规范和完善中医辨治体系,更好地发挥中医优势特色。As the third most common chronic airway disease,bronchiectasis is often neglected throughout its management,which may exacerbate acute exacerbations and pulmonary function decline,resulting in a heavy disease burden.This article reviewed the current status of prevention and treatment of bronchiectasis,the theoretical understanding of TCM,and the progress of TCM staging and phenotype treatment of bronchiectasis.It was believed that in different stages of disease and different clinical phenotypes,integrated traditional Chinese and Western medicine and the combination of Chinese materia medica and Western medicine could better improve symptoms,delay the decline of lung function,promote the absorption of lung lesions and reduce acute exacerbations,giving full play to the unique advantages of TCM in treating pre-disease,syndrome differentiation and treatment,and multi-target intervention,and participating in the whole process of disease management,which would help to construct a combined treatment plan of chronic lung disease with TCM and Western medicine.It was necessary to actively explore the efficacy characteristics of TCM on different bronchiectasis phenotypes,especially refractory bronchiectasis(secondary microbial colonization,repeated acute exacerbation phenotype,with repeated hemoptysis,etc.),accumulate clinical and research data,and standardize and improve TCM system of differentiation and treatment,in order to give better play to the advantages and characteristics of TCM.
