内镜下食管扩张术对不同病因所致食管狭窄的临床疗效

摘　　要：目的 探讨Savary-Gilliard探条食管扩张术(莎氏扩张术)对不同病因所致食管狭窄的临床疗效。方法 选取2017年7月—2021年12月于徐州医科大学附属第三医院行莎氏扩张术的食管狭窄患者60例,收集临床资料进行回顾性分析。其中食管癌术后食管胃吻合口狭窄32例,食管癌放疗后伴狭窄15例,食管烧伤伴狭窄13例。评估患者行莎氏扩张术后吞咽困难改善情况。根据术后患者进食情况判断疗效。记录患者扩张次数、手术前后狭窄部位直径、术后并发症情况。结果 经莎氏扩张术治疗后,不同病因食管狭窄患者吞咽困难得到不同程度的缓解(P≤0.001),显效率95%,总有效率100%,术后2个月随访总有效率87.3%。食管狭窄处直径明显增宽(P<0.001),术后未出现并发症。结论 内镜下莎氏扩张术疗效确切,对不同原因导致的食管狭窄,均能取得较好效果。Objective To explore the clinical effectiveness of Savary-Gilliard esophageal dilatation on esophageal strictures caused by different etiologies.Methods A total of 60 patients with esophageal strictures who were admitted to the Third Affiliated Hospital of Xuzhou Medical University from July 2017 to December 2021 and underwent Savary-Gilliard esophageal dilatation were enrolled and their clinical data were retrospectively analyzed.There were 32 cases of esophagogastric anastomotic stenosis after esophageal cancer surgery,15 cases of esophageal cancer with stenosis after radiotherapy,and 13 cases of esophageal burn with stenosis.Their dysphagia and postoperative feeding were evaluated.The number of dilation times,the diameter of esophageal strictures before and after operation,and postoperative complications were recorded.Results After esophageal dilatation,dysphagia was relieved to various extent in patients with esophageal strictures caused by different etiologies(P≤0.001),with an obviously effective rate of 95%and a total effective rate of 100%.According to follow-up visits at postoperative two months,the total effective rate became 87.3%.The diameter of esophageal strictures were markedly widened(P<0.001),and no postoperative complication was reported.Conclusions Savary-Gilliard esophageal dilatation has definite clinical effectiveness,and can well relieve esophageal strictures caused by different etiologies.
