雷火灸联合药物治疗心肾阳虚型Ⅱ型心肾综合征的疗效观察及对血清半乳凝素-3和趋化素水平的影响

摘　　要：目的观察雷火灸联合药物治疗心肾阳虚型Ⅱ型心肾综合征的临床疗效及对患者血清半乳凝素-3和趋化素的影响。方法将72例心肾阳虚型Ⅱ型心肾综合征患者随机分为对照组和观察组,每组36例。对照组予常规药物治疗,观察组予雷火灸联合常规药物治疗。比较两组治疗前后心功能指标[每搏输出量、左室射血分数以及血清N端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)水平]、肾功能指标(肾动脉内径、肾动脉收缩期峰值流速、血肌酐和尿素氮)以及血清半乳凝素-3和趋化素水平。比较两组临床疗效。结果观察组总有效率高于对照组(P<0.05)。治疗后,两组每搏输出量和左室射血分数均升高(P<0.05),血清NT-proBNP水平均降低(P<0.05);且观察组上述心功能指标均优于对照组(P<0.05)。治疗后,两组肾动脉内径、肾动脉收缩期峰值流速均升高(P<0.05),血肌酐和尿素氮水平均降低(P<0.05);且观察组上述肾功能指标均优于对照组(P<0.05)。治疗后,两组血清半乳凝素-3和趋化素水平均降低(P<0.05),且观察组低于对照组(P<0.05)。结论雷火灸联合药物治疗心肾阳虚型Ⅱ型心肾综合征的疗效优于单一药物治疗,可进一步改善心肾功能,可能与降低患者血清半乳凝素-3和趋化素水平有关。Objective To observe the clinical efficacy of thunder-fire moxibustion plus medication in treating cardio-renal syndrome type 2 due to Yang deficiency of the heart and kidney pattern and its effects on the patient’s serum galectin-3 and chemerin levels.Method Seventy-two patients with cardio-renal syndrome type 2 due to Yang deficiency of the heart and kidney pattern were randomized into a control group and an observation group,with 36 cases in each group.The control group received the conventional pharmaceutical treatment,and the observation group received thunder-fire moxibustion in combination with the conventional pharmaceutical treatment.Before and after the intervention,the following items were compared,including the cardiac function indicators[stroke volume,left ventricular ejection fraction,and serum N-terminal pro-brain natriuretic peptide(NT-proBNP)],kidney function indexes(renal artery diameter,renal artery peak systolic velocity,serum creatinine,and urea nitrogen),and the levels of serum galectin-3 and chemerin.The clinical efficacy was also compared between the two groups.Result The total effective rate was higher in the observation group than in the control group(P<0.05).After the treatment,the stroke volume and left ventricular ejection fraction increased in both groups(P<0.05),and the serum NT-proBNP level dropped(P<0.05);the observation group outperformed the control group in comparing the cardiac function indicators mentioned above(P<0.05).After the intervention,the renal artery diameter and peak systolic velocity were heightened in both groups(P<0.05),and the levels of serum creatinine and urea nitrogen decreased(P<0.05);the observation group was superior to the control group in comparing these kidney function indexes(P<0.05).The levels of serum galectin-3 and chemerin dropped after the treatment in the two groups(P<0.05)and were lower in the observation group than in the control group(P<0.05).Conclusion Thunder-fire moxibustion plus medication can produce more significant efficacy than medic
