摘　　要：目的探讨巧克力球囊治疗股腘动脉粥样硬化闭塞症的安全性及有效性。方法收集2021年3—7月于郑州大学第五附属医院采用巧克力球囊治疗的53例股腘动脉粥样硬化闭塞症患者的临床资料,观察其术后手术成功率及术后随访12个月时的一期通畅率、免于临床驱动靶病变血运重建(CD-TLR)发生率。结果53例患者手术成功率为98.1%(52/53),均未发生截肢、死亡、限流性夹层,50例患者完成12个月随访。随访结束时,踝肱指数(ABI)为(0.89±0.13),高于术前的(0.46±0.11),差异有统计学意义(P﹤0.05)。Kaplan-Meier生存分析显示,53例患者一期通畅率为70.9%,免于CD-TLR发生率为81.1%。随访结束时,不同钙化分型患者一期通畅率比较,差异均无统计学意义(P﹥0.05)。结论巧克力球囊治疗股腘动脉粥样硬化闭塞症安全、有效,值得临床上推广应用。Objective To investigate the safety and efficacy of chocolate balloon in femoral popliteal atherosclerotic occlusion.Method Clinical data of 53 patients with femoral popliteal atherosclerotic occlusion treated with chocolate balloon from March to July 2021 in the Fifth Affiliated Hospital of Zhengzhou University were collected,the successful rate of operation,primary patency rate at 12 months after operation and rate of patient free from clinically driven target lesion reintervention(CD-TLR)were observed.Result The successful rate of operation was 98.1%(52/53)in 53 patients,without amputation,death and flow limiting dissection,and 50 patients completed 12-month follow up.At the end of follow up,the ankle brachial index(ABI)was(0.89±0.13),higher than that before operation with(0.46±0.11),the difference was statistically significant(P<0.05).Kaplan-Meier survival analysis showed the primary patency rate of 53 patients was 70.9%,and the rate of patient free from CD-TLR was 81.1%.At the end of follow up,there was no difference for the primary patency rate of different calcification types(P>0.05).Conclusion It is safe and effective to treat femoral popliteal atherosclerotic occlusion with chocolate balloon,which is worthy of clinical application.
