益肾健脾补血方联合促红细胞生成素治疗维持性血液透析肾性贫血临床观察

摘　　要：目的:观察益肾健脾补血方联合促红细胞生成素治疗维持性血液透析肾性贫血的临床疗效。方法:选取2019年9月至2021年8月兴国县中医院诊治的维持性血液透析肾性贫血患者80例,采用信封随机化分组法将患者均分为对照组和观察组,每组40例。对照组给予促红细胞生成素治疗,观察组在对照组治疗的基础上加用益肾健脾补血方治疗。观察两组患者临床疗效、不良反应发生率、生活质量评分与治疗前后血红细胞参数[血红蛋白(hemoglobin, Hb)、白蛋白(albumin, ALB)、铁蛋白(ferritin, SF)]。结果:观察组有效率为97.50%,对照组有效率为80.00%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率5.00%,对照组不良反应发生率10.00%,观察组不良反应发生率低于对照组,但差异无统计学意义(P>0.05);观察组生活质量评分高于对照组,差异有统计学意义(P<0.05);观察组治疗后Hb、ALB、SF高于对照组,差异有统计学意义(P<0.05)。结论:益肾健脾补血方联合促红细胞生成素治疗维持性血液透析肾性贫血,可提高临床疗效,改善患者生活质量水平和贫血症状,且不良反应发生率低。Objective:To observe the clinical efficacy of Decoction for Replenishing the Kidney and Invigorating the Spleen and Tonifying Blood combined with erythropoietin on renal anemia in maintenance hemodialysis.Methods:A total of 80 patients with renal anemia in maintenance hemodialysis treated in Xingguo County Hospital of Traditional Chinese Medicine from September 2019 to August 2021 were randomly divided into the control group and the observation group by envelope randomization,with 40 cases in each group.The control group was treated with erythropoietin,while the observation group was treated with Decoction for Replenishing the Kidney and Invigorating the Spleen and Tonifying Blood on the basis of the treatment of the control group.The clinical efficacy,incidence of adverse reactions,quality of life score and the changes of blood red blood cell parameters[including hemoglobin(Hb),albumin(ALB),and ferritin(SF)]of the two groups before and after treatment were observed.Results:The effective rate of the observation group was 97.50%,which was higher than that of the control group;80.00%,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.00%,which was lower than that of the control group;10.00%,but the difference was not statistically significant(P>0.05).The quality of life score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,Hb,ALB and SF in the observation group were significantly higher than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Decoction for Replenishing the Kidney and Invigorating the Spleen and Tonifying Blood combined with erythropoietin can improve the clinical efficacy,quality of life and anemia symptoms of patients with renal anemia in maintenance hemodialysis,and the incidence of adverse reactions is low.
