天麻钩藤饮加减联合针刺治疗肝阳上亢型偏头痛临床观察

摘　　要：目的:观察天麻钩藤饮加减联合针刺治疗肝阳上亢型偏头痛的临床疗效。方法:选取2020年1月至2021年12月鹤壁市中医院收治的肝阳上亢型偏头痛患者100例,按照随机数字表法分为观察组和对照组,每组50例。对照组给予针刺治疗,观察组在对照组治疗的基础上给予天麻钩藤饮加减治疗。观察两组患者临床疗效、头痛症状控制情况、脑部血流情况[大脑前动脉(anterior cerebral artery, ACA)、大脑中动脉(middle cerebral artery, MCA)、大脑后动脉(posterior cerebral artery, PCA)、椎动脉(vertebral artery, VA)、基底动脉(basilar artery, BA)平均血流速度]、偏头痛生活质量特异性等指标。结果:观察组有效率为96.0%,对照组有效率为80.0%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后头痛发作次数评分、头痛程度评分、头痛持续时间评分、伴随症状评分及中医证候积分低于对照组,差异有统计学意义(P<0.05);观察组治疗后ACA、MCA、PCA、VA、BA平均血流速度均低于对照组,差异有统计学意义(P<0.05);观察组治疗后功能受限、功能丧失、情感功能及总积分低于对照组,差异有统计学意义(P<0.05)。结论:天麻钩藤饮加减联合针刺治疗肝阳上亢型偏头痛,可减轻患者疼痛程度、头痛持续时间、伴随症状,改善临床症状。Objective:To observe the clinical efficacy of Modified Gastrodia and Uncaria Decoction combined with acupuncture on migraine with syndrome of liver-yang hyperactivity.Methods:A total of 100 migraine patients with syndrome of liver-yang hyperactivity admitted to Hebi Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were randomly divided into the observation group and the control group,with 50 cases in each group.The control group was treated with acupuncture,while the observation group was treated withModified Gastrodia and Uncaria Decoction combined with acupuncture.The clinical efficacy,headache symptom control,cerebral blood flow conditions[including average blood flow velocity of anterior cerebral artery(AC A),middle cerebral artery(MCA),posterior cerebral artery(PCA),vertebral artery(VA),basilar artery(BA)],and the specificity of migraine quality of life were observed in the two groups.Results:The effective rate of the observation group was 96.0%,while that of the control group was 80.0%.The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the score of headache attack frequency,headache degree,headache duration,accompanying symptom score,and TCM syndrome scores in the observation group were lower than those in the control group,and all the differences were statistically significant(P<0.05);Each average blood flow velocity of ACA,MCA,PCA,VA,and BA in the observation group was lower than that in the control group,and all the differences were statistically significant(P<0.05);The observation group showed significantly lower functional limitations,loss of function,emotional function,and total score compared to the control group,and all the differences were statistically significant(P<0.05).Conclusion:Modified Gastrodia and Uncaria Decoction combined with acupuncture can reduce the degree of pain,duration of headache,accompanying symptoms,and improve clinical symptoms in patients wit
