摘 要:火把花根片(colquhounia root tablet, CRT)治疗2型糖尿病(type 2 diabetes mellitus, T2DM)蛋白尿和肾脏损伤临床疗效良好,其治疗2型糖尿病肾病的药理机制有减轻炎症损伤、抗肾小球纤维化、足细胞保护作用、抗氧化应激、保护肾小管上皮细胞、改善脂质代谢等。但火把花根片相关临床研究仍存在以下不足之处:(1)CRT治疗T2DKD的量效关系和药理毒理机制尚未完全阐明,多项研究样本量不足,研究中存在各种偏倚因素,导致循证医学证据级别不高。未来需要大样本高质量的随机对照临床试验来验证CRT在T2DKD患者中的疗效和安全性。(2)CRT的毒副作用以消化道反应最常见,还有一定肝肾损害及生殖毒性,虽然未见严重不良事件,但仍制约了CRT的广泛临床使用。如何能够实现CRT减毒增效或个体化的临床应用,也将成为T2DKD研究领域的重要课题。Colquhounia root tablet(CRT)has good clinical efficacy in the treatment of proteinuria and kidney injury in type 2 diabetes mellitus(T2DM).Its pharmacological mechanisms in the treatment of type 2 diabetic kidney disease(T2DKD)include reducing inflammatory damage,anti-glomerular fibrosis,foot cytoprotection,anti-oxidant stress,protecting renal tubular epithelial cells,improving lipid metabolism,etc.However,there are still shortcomings in clinical research related to CRT:①The dose-effect relationship and pharmacological and toxicological mechanisms of CRT treatment for T2DKD have not been fully elucidated,and multiple studies have insufficient sample sizes.There are various bias factors in the studies,resulting in a low level of evidencebased medicine evidence.In the future,high-quality randomized controlled clinical experiments with large samples are needed to verify the efficacy and safety of CRT in T2DKD patients.②Gastrointestinal reactions are the most common side effects of CRT,as well as some liver and kidney damage and reproductive toxicity.Although no serious adverse events are found,it still restricts the extensive clinical use of CRT.How to achieve the clinical application of CRT in reducing toxicity and increasing efficiency or individualization will also become an important topic in the field of T2DKD research.
