磁共振扩散加权成像在慢性肾病肾功能评价中的应用

摘　　要：目的研究磁共振扩散加权成像在慢性肾病肾功能评价中的应用。方法选取2021年3月-2022年3月在我院诊治的74例慢性肾病患者为研究对象设为观察组,并选取同期74例在我院体检健康者为对照组,均进行磁共振扩散加权成像,比较两组患者表观扩散系数(ADC)、慢性肾病患者左右肾ADC值、不同分期慢性肾病患者和不同肾小球滤过率(GFR值)以及血清肌酐(SCr水平)患者与对照组ADC值。结果观察组ADC值低于对照组(P<0.05);右肾ADC值与左肾比较,差异无统计学意义(P>0.05);Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期、Ⅴ期患者ADC值均低于对照组,同时Ⅰ期ADC均高于Ⅱ期、Ⅲ期、Ⅳ期、Ⅴ期,且随分期升高,ADC值越低(P<0.05);GFR轻度、中度、重度患者ADC值均低于对照组(P<0.05);GFR轻度患者ADC值均高于中度、重度患者,但差异无统计学意义(P>0.05);SCr升高患者ADC值低于对照组(P<0.05);SCr正常患者ADC值与对照组比较,差异无统计学意义(P>0.05)。结论磁共振扩散加权成像在慢性肾病肾功能评价中具有重要的价值,可通过ADC值判断慢性肾病分期、肾功能损伤程度,对临床病情判断、对症治疗具有积极的作用。Objective To study the application of diffusion weighted imaging(DWI)in evaluation of renal function in patients with chronic kidney disease(CKD).Methods A total of 74 patients with chronic kidney disease who were diagnosed and treated in our hospital from March 2021 to March 2022 were selected as the observation group,and 74 healthy subjects who underwent physical examination in our hospital during the same period were selected as the control group.Diffusion weighted imaging was performed to compare the apparent diffusion coefficient(ADC)of the two groups,the ADC values of the left and right kidneys of patients with chronic kidney disease,patients with different stages of chronic kidney disease and patients with different glomerular filtration rates(GFR values)and serum creatinine(SCr levels)and the ADC values of the control group.Results The ADC value of the observation group was lower than that of the control group(P<0.05).There was no significant difference in ADC value between the right kidney and the left kidney(P>0.05).The ADC values of stage Ⅰ,stage Ⅱ,stage Ⅲ,stage Ⅳ and stage Ⅴ patients were lower than those of the control group,and the ADC values of stage Ⅰ were higher than those of stage Ⅱ,stage Ⅲ,stage Ⅳ and stage Ⅴ,and the higher the stage,the lower the ADC value(P<0.05).The ADC values of patients with mild,moderate and severe GFR were lower than those of the control group(P<0.05).The ADC values of patients with mild GFR were higher than those of patients with moderate and severe GFR,but the difference was not statistically significant(P>0.05).The ADC value of patients with elevated SCr was lower than that of the control group(P<0.05).There was no significant difference in ADC value between patients with normal SCr and control group(P>0.05).Conclusion Diffusion weighted imaging is of great value in the evaluation of renal function of chronic kidney disease.Meanwhile,ADC value can be used to judge the stage of chronic kidney disease and the degree of renal function injury,which has
