AngioJet血栓抽吸术与置管溶栓术治疗深静脉血栓临床疗效和凝血功能的比较

摘　　要：目的对比AngioJet血栓抽吸术(PMT)与置管溶栓术(CDT)治疗深静脉血栓(DVT)的临床疗效与凝血功能。方法选取佳木斯市中心医院周围血管外科2019年10月-2021年10月收治的64例DVT患者,按照随机数字表法分为PMT组(32例)与CDT组(32例)。PMT组行AngioJet血栓抽吸术治疗,CDT组行置管溶栓术治疗,比较两组临床疗效(血栓清除率、患肢消肿率)、凝血功能[D二聚体(D-D)、纤维蛋白原(Fib)]、临床指标(尿激酶使用剂量、溶栓时间、住院时间)、并发症、患者耐受情况及血栓复发情况。结果两组血栓清除率、患肢消肿率比较,差异无统计学意义(P>0.05);两组治疗后D-D、Fib低于治疗前(P<0.05),但组间比较,差异无统计学意义(P>0.05);PMT组尿激酶用量少于CDT组,且溶栓时间及住院时间均短于CDT组(P<0.05);PMT组并发症发生率低于CDT组,患者耐受优良率高于CDT组(P<0.05);治疗后6个月,两组血栓复发率比较,差异无统计学意义(P>0.05)。结论PMT与CDT治疗DVT疗效相当,二者均可改善患者凝血功能,且复发率低,但PMT溶栓药物剂量更小、溶栓时间更短、术后恢复更快、并发症更少,患者耐受度相对更好。Objective To compare the clinical efficacy and coagulation function of AngioJet thrombus aspiration(PMT)and catheter thrombolysis(CDT)in the treatment of deep vein thrombosis(DVT).Methods A total of 64 patients with DVT admitted to the Department of Peripheral Vascular Surgery,Jiamusi Central Hospital from October 2019 to October 2021 were selected and divided into PMT group(32 cases)and CDT group(32 cases)according to the random number table method.The PMT group was treated with AngioJet thrombus aspiration,and the CDT group was treated with catheter thrombolysis.The clinical efficacy(thrombus clearance rate,limb swelling rate),coagulation function[D-dimer(D-D),fibrinogen(Fib)],clinical indicators(urokinase dosage,thrombolysis time,hospitalization time),complications,patient tolerance and thrombus recurrence were compared between the two groups.Results There was no significant difference in thrombus clearance rate and limb swelling rate between the two groups(P>0.05).After treatment,D-D and Fib in the two groups were lower than those before treatment(P<0.05),while there was no significant difference between the two groups(P>0.05).The dosage of urokinase in the PMT group was less than that in the CDT group,and the thrombolysis time and hospitalization time were shorter than those in the CDT group(P<0.05).The incidence of complications in the PMT group was lower than that in the CDT group,and the excellent and good rate of tolerance was higher than that in the CDT group(P<0.05).At 6 months after treatment,there was no significant difference in the recurrence rate of thrombosis between the two groups(P>0.05).Conclusion PMT and CDT have similar efficacy in the treatment of DVT.Both of them can improve the coagulation function of patients,and the recurrence rate is low.However,PMT has smaller thrombolytic dose,shorter thrombolytic time,faster postoperative recovery,fewer complications,and better patient tolerance.
