自拟疏肝健脾补肾活血汤联合皙毓黄褐斑贴敷料治疗黄褐斑患者的效果

摘　　要：目的:观察自拟疏肝健脾补肾活血汤联合皙毓黄褐斑贴敷料治疗黄褐斑患者的效果。方法:选取2019年1月至2022年1月该院皮肤科收治的80例黄褐斑患者进行前瞻性研究,采用随机数字表法分为对照组和观察组各40例。对照组采用皙毓黄褐斑贴敷料治疗,观察组在对照组基础上联合自拟疏肝健脾补肾活血汤治疗。比较两组治疗前后中医证候积分、治疗效果、治疗前后皮损恢复情况[黄褐斑面积及严重度指数(MASI)]评分和治疗期间不良反应发生率。结果:治疗后,观察组颜面黄斑、双膝酸软、急躁易怒和胸肋胀痛等中医证候积分均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率为97.50%(39/40),高于对照组的80.00%(32/40),差异有统计学意义(P<0.05);治疗后,观察组MASI评分均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:自拟疏肝健脾补肾活血汤联合皙毓黄褐斑贴敷料治疗黄褐斑患者可提高治疗总有效率,降低中医证候积分和MASI评分,效果优于单纯皙毓黄褐斑贴敷料治疗。Objective:To observe effects of self-made Shugan Jianpi Bushen Huoxue decoction combined with Xiyu chloasma patch dressing on patients with chloasma.Methods:A prospective study was conducted on 80 patients with chloasma admitted to the dermatology department of the hospital from January 2019 to January 2022.They were divided into control group and observation group by using the random number table method,40 cases in each.The control group was treated with Xiyu chloasma patch dressing,while the observation group was treated with self-made Shugan Jianpi Bushen Huoxue decoction on the basis of that of the control group.The TCM syndrome score,the therapeutic effects,the recovery of skin lesions[melasma area and severity index(MASI)]scores before and after treatment,and the incidence of adverse reactions during the treatment were compared between the two groups.Results:After the treatment,the scores of TCM syndromes such as facial macular,soreness and weakness of both knees,irritability and chest pain in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 97.50%(39/40),which was higher than 80.00%(32/40)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the MASI scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Self-made Shugan Jianpi Bushen Huoxue decoction combined with Xiyu chloasma patch dressing in the treatment of the chloasma patients can improve the total effective rate of treatment and reduce the TCM syndrome scores and the MASI scores.Moreover,it is superior to single Xiyu chloasma patch dressing treatment.
