InterTan钉与Gamma3钉内固定术治疗老年股骨转子间骨折患者的效果比较

摘　　要：目的:比较InterTan钉与Gamma3钉内固定术治疗老年股骨转子间骨折患者的效果。方法:选取2020年1月至2021年12月该院收治的120例老年股骨转子间骨折患者进行前瞻性研究,按照随机数字表法分为Gamma3组与InterTan组各60例。Gamma3组采用Gamma3钉内固定术治疗,InterTan组采用InterTan钉内固定术治疗,比较两组手术相关指标水平、临床骨折愈合时间、住院时间、术后首次下床时间、髋关节功能评分量表(Harris)评分、Berg平衡量表(BBS)评分、改良Barthel指数(MBI)评分和并发症发生率。结果:Gamma3组术中出血量少于InterTan组,手术时间短于InterTan组,差异均有统计学意义(P<0.05);术后3个月,InterTan组Harris评分高于Gamma3组,差异有统计学意义(P<0.05);两组术中透视次数、临床骨折愈合时间、住院时间、术后首次下床时间、BBS评分、MBI评分和并发症发生率比较,差异均无统计学意义(P>0.05)。结论:InterTan钉内固定术治疗老年股骨转子间骨折患者提高Harris评分的效果优于Gamma3钉内固定术,但会增加术中出血量和延长手术时间。Objective:To compare effects of InterTan and Gamma3 nail internal fixation in treatment of elderly patients with femur intertrochanteric fractures.Methods:A prospective study was conducted on 120 elderly patients with femur intertrochanteric fractures admitted to the hospital from January 2020 to December 2021.According to the random number table method,they were divided into Gamma3 group and InterTan group,60 cases in each.The Gamma3 group was treated with Gamma3 nail internal fixation,while the InterTan group was treated with InterTan nail internal fixation.The levels of surgical related indicators,the clinical fracture healing time,the hospitalization time,the time to first get out of bed after the surgery,the Harris score,the Berg balance scale(BBS)score,the modified Barthel index(MBI)score,and the incidence of complications were compared between the two groups.Results:The intraoperative blood output in the Gamma3 group was lower than that in the InterTan group,the operation time was shorter than that in the InterTan group,and the differences were statistically significant(P<0.05).3 months after the surgery,the Harris score in the InterTan group was higher than that in the Gamma3 group,and the difference was statistically significant(P<0.05).There were no statistically significant differences between the two groups in terms of intraoperative fluoroscopy frequency,clinical fracture healing time,hospitalization time,the time to first get out of bed after the surgery,BBS score,MBI score,and incidence of complications(P>0.05).Conclusions:InterTan nail internal fixation can improve the Harris scores in the elderly patients with femur intertrochanteric fractures,which is better than Gamma3 nail internal fixation,but it can increase the intraoperative bleeding and prolong the operation time.
