苁蓉益肾颗粒对多囊卵巢综合征患者内分泌功能、血清淀粉样蛋白P及白细胞介素-18水平的影响

摘　　要：目的 探讨苁蓉益肾颗粒对多囊卵巢综合征(Polycystic Ovary Syndrome, PCOS)患者内分泌功能、血清淀粉样蛋白P(Serum Amyloid P,SAP)、白细胞介素-18(Interleukin-18,IL-18)水平的影响。方法 选取2018年6月—2020年6月期间河南省周口市中医院收治的90例多囊卵巢综合征患者,采用随机摸球法分为对照组和观察组,每组各45例。对照组采用氯米芬治疗,观察组在治疗组的基础上加用苁蓉益肾颗粒治疗。治疗2个疗程后,观察比较两组患者临床疗效、卵巢B超结果、内分泌功能指标、SAP及IL-18水平。结果 治疗后观察组总有效率93.33%(42/45)明显优于对照组73.33%(33/45),差异有统计学意义(P<0.05)。治疗3个周期后两组患者平均卵泡数、平均卵巢体积均较治疗前降低,差异有统计学意义(P<0.05);且观察组平均卵泡数、平均卵巢体积均低于对照组,差异有统计学意义(P<0.05)。停药后第3个周期观察组平均卵泡数及平均卵巢体积与对照组比较,差异有统计学意义(P<0.05)。治疗后两组患者血清雌二醇(Estradiol, E2)、睾酮(Testosterone, T)、黄体生成素(Luteinizing hormone, LH)水平均较治疗前降低,FSH水平均较治疗前升高,差异有统计学意义(P<0.05);且观察组E2、T、LH水平均低于对照组,FSH水平高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清IL-18、SAP水平均较治疗前降低,差异有统计学意义(P<0.05);且观察组血清IL-18、SAP水平均低于对照组,差异有统计学意义(P<0.05)。结论 苁蓉益肾颗粒治疗PCOS的效果显著,能明显纠正患者内分泌失调,降低血清IL-18及SAP水平。Objective To investigate the effect of Congrong Yishen Granules on endocrine function and levels of serum amyloid P component(SAP)and interleukin-18(IL-18)in patients with polycystic ovary syndrome(PCOS).Methods A total of 90 patients with PCOS who were treated in Zhoukou Hospital of Traditional Chinese Medicine from June 2018 to June 2020 were randomized into control group(n=45)and observation group(n=45)with the ball-touching method.The control group was treated with clomiphene,and the observation group with Congrong Yishen Granules and clomiphene.After two courses of treatment,the clinical efficacy,B-ultrasound result of the ovary,endocrine function indexes,and levels of SAP and IL-18 were compared between the two groups.Results After treatment,the total effective rate of the observation group was 93.33%(42/45),as compared with the 73.33%(33/45)of the control group(P<0.05).After three cycles of treatment,the average number of follicles and average ovarian volume were smaller than those before treatment in the two groups(P<0.05)and they were smaller in the observation group than in the control group(P<0.05).The average number of follicles and the average ovarian volume in the observation group were significantly different from those in the control group in the third cycle after drug withdrawal(P<0.05).After treatment,the levels of estradiol(E2),testosterone(T),and luteinizing hormone(LH)were lower than those before treatment and the level of FSH was higher than that before treatment in the two groups(P<0.05).The levels of E2,T,and LH in the observation group were lower than those in the control group and the level of FSH was higher than that in the control group(P<0.05).After treatment,the levels of serum IL-18 and SAP were lower than those before treatment in the two groups(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).Conclusion Congrong Yishen Granules shows remarkable efficacy in the treatment of PCOS,which can obviously alleviate the endocrine disorders
