摘　　要：目的比较反复种植失败(RIF)患者和经产妇黄体中期外周血和子宫内膜组织CD56^(+)自然杀伤(NK)细胞和CD16+NK细胞的差异,探讨RIF患者全身和局部免疫的特点。方法选取2018年1月至12月在广西壮族自治区妇幼保健院行体外受精(IVF)助孕的RIF患者作为RIF组(20例),经IVF助孕成功分娩1年以上的患者作为对照组(20例),取两组患者黄体中期的外周血和子宫内膜组织,使用流式细胞术测定两组外周血CD56^(+)NK细胞的比率,并使用免疫组织化学方法检测两组子宫内膜CD56^(+)NK细胞和CD16+NK细胞的表达。比较两组患者的外周血CD56^(+)NK细胞的比率和子宫内膜CD56^(+)NK细胞和CD16+NK细胞的表达。结果RIF患者黄体中期外周血CD56^(+)NK细胞比率高于正常组,子宫内膜CD56^(+)NK细胞比率低于正常组,CD16+NK细胞比率高于正常组,差异有统计学意义(P<0.05)。结论外周血检测NK细胞比率可能反映子宫内膜局部的免疫状态。Objective To compare CD56^(+)natural killer(NK)cells and CD16+NK cells in peripheral blood and endometrium of patients with repeated implantation failure(RIF)and multiparas,and to explore the systemic and local immune characteristics of RIF patients.Methods Patients with RIF who underwent in vitro fertilization(IVF)in Maternal and Child Health Care of Guangxi Zhuang Autonomous Region from January to December 2018 were selected as RIF group(n=20),and patients who successfully delivered after IVF for more than 1 year were selected as control group(n=20).Peripheral blood and endometrial tissue in the mid-luteal phase of the two groups were collected.The proportion of CD56^(+)NK cells in peripheral blood was measured by flow cytometry.The expression of CD56^(+)NK cells and CD16+NK cells in endometrium were detected by immunohistochemistry.The ratio of CD56^(+)NK cells in peripheral blood and the expression of CD56^(+)NK cells and CD16+NK cells in endometrium were compared between the two groups.Results The proportion of CD56^(+)NK cells in the peripheral blood of patients with RIF in mid-luteal phase was higher than that of the normal group,the proportion of CD56^(+)NK cells in the endometrium was lower than that of the normal group,and the proportion of CD16+NK cells was higher than that of the normal group,the difference was statistically significant(P<0.05).Conclusion The percentage of NK cells in peripheral blood may reflect the local immune status of the endometrium.
