双胎早产儿坏死性小肠结肠炎的危险因素分析

摘　　要：目的探讨双胎早产儿发生坏死性小肠结肠炎(NEC)的危险因素。方法回顾性选取2010年1月至2021年12月福建省妇幼保健院新生儿科收治的67对双胎早产儿作为研究对象,其中双胎之一患NEC(BellⅡ期及以上),另一个双胎未患NEC。按照是否患NEC分为NEC组和对照组,比较两组发生NEC的潜在危险因素,包括基本信息(出生胎龄、出生体重、出生顺序、性别)和危险因素(围生期危险因素及合并症、主要治疗措施、喂养情况)。结果单因素分析结果显示,NEC与先天性心脏病、小于胎龄儿、出生时轻度窒息、喂养不耐受有关,差异有统计学意义(P<0.05)。logistic回归模型进行多因素分析结果显示,喂养不耐受(OR=5.786,95%CI:1.839~18.198,P=0.003)是双胎早产儿发生NEC的独立危险因素。结论喂养不耐受是双胎早产儿发生NEC的危险因素。Objective To analyze the risk factors of necrotizing enterocolitis(NEC)in twin preterm infants.Methods A total of 67 pairs of twin preterm infants admitted to the Department of Neonatology,Fujian Maternity and Child Health Hospital from January 2010 to December 2021 were retrospectively selected as the research objects.One of the twins had NEC(Bell stageⅡor above),and the other twin did not have NEC.They were divided into NEC group and control group according to whether they had NEC.The potential risk factors of NEC of the two groups were compared,including basic information(gestational age at birth,birth weight,birth order,sex)and risk factors(perinatal risk factors and comorbidities,main treatment measures and feeding).Results Univariate analysis showed that NEC was associated with congenital heart disease,small for gestational age,mild asphyxia at birth and feeding intolerance,and the differences were statistically significant(P<0.05).Multivariate analysis by logistic regression model showed that feeding intolerance(OR=5.786,95%CI:1.839-18.198,P=0.003)was an independent risk factor for NEC in twin preterm infants.Conclusion Feeding intolerance is a risk factor for NEC in twin preterm infants.
