摘 要:目的研究2019—2020年北京市丰台中西医结合医院尿细菌培养情况及药敏试验结果。方法回顾性分析2019年1月至2020年12月北京市丰台中西医结合医院送检的1930例尿液标本,所有尿液标本均接受病原菌培养,统计分析病原菌分布比例与药敏试验结果。结果1930例尿液标本中共培养出病原菌1613株(83.58%),包括革兰氏阴性菌1273株(78.92%),革兰氏阳性菌267株(16.55%),真菌73株(4.53%)。革兰氏阴性菌中以大肠埃希菌(63.05%)为主;革兰氏阳性菌中以屎肠球菌(8.25%)、粪肠球菌(5.21%)为主;真菌包括热带念珠菌(3.04%)、白色念珠菌(1.49%)两种。病原菌分布科室以内科(41.41%)、泌尿科(40.36%)为主。革兰氏阴性菌中大肠埃希菌对氨苄西林、复方新诺明、环丙沙星、头孢唑林的耐药率均>50%,同时对头孢曲松、左氧氟沙星耐药率接近50%;肺炎克雷伯菌对头孢唑林的耐药率>40%。革兰氏阳性菌中屎肠球菌对氨苄西林、红霉素、环丙沙星、莫西沙星、青霉素、左氧氟沙星的耐药率均>90%;粪肠球菌对四环素的耐药率>80%,对红霉素的耐药率>70%。大肠埃希菌中超广谱β-内酰胺酶菌群(ESBLs)阳性株数为446株(43.85%);肺炎克雷伯菌中ESBLs阳性株数为31株(26.46%)。结论2019—2020年北京市丰台中西医结合医院送检的尿液标本中以革兰氏阴性菌为主,主要分布在内科和泌尿科,应结合不同细菌的耐药率检测结果合理使用抗生素。Objective To study the urine bacterial culture and drug sensitivity test results from 2019 to 2020 in Fengtai Hospital of Integrative Medicine in Beijing.Methods The 1930 urine specimens sent to Beijing Fengtai Hospital of Integrative Medicine from January 2019 to December 2020 were retrospectively analyzed,and all urine specimens were subjected to pathogenic bacteria culture,and the proportion of pathogenic bacteria distribution and drug sensitivity test results were statistically analyzed.Results A total of 1613 pathogenic bacteria(83.58%)were cultured in 1930 urine specimens,including 1273 Gram-negative(78.92%),267 Gram-positive(16.55%),and 73 fungal(4.53%)strains.Among the Gram-negative bacteria,Escherichia coli(63.05%)was predominant.Among the Gram-positive bacteria,Enterococcus faecalis(8.25%)and Enterococcus faecalis(5.21%)were predominant.The fungi included Candida tropicalis(3.04%)and Candida albicans(1.49%).The pathogenic bacteria distribution departments were mainly internal medicine(41.41%)and urology(40.36%).Among the Gram-negative bacteria,the resistance rate of Escherichia coli to Ampicillin,Cotrimoxazole,Ciprofloxacin and Cefazolin were>50%,while the resistance rate to Ceftriaxone and Levofloxacin was close to 50%.The resistance rate of Klebsiella pneumoniae to Cefazolin was>40%.Among the Gram-positive bacteria,the resistance rates of Enterococcus faecalis to Ampicillin,Erythromycin,Ciprofloxacin,Moxifloxacin,Penicillin and Levofloxacin were>90%.The resistance rates of Enterococcus faecalis to Tetracycline were>80%and to Erythromycin were>70%.The number of Extended-spectrumβ-lactamase bacteria(ESBLs)positive strains in Escherichia coli was 446(43.85%).The number of ESBLs positive strains in Klebsiella pneumoniae was 31(26.46%).Conclusion In 2019—2020,the urine specimens sent to Fengtai Hospital of Integrative Medicine in Beijing in combination with hospitals are mainly Gram-negative bacteria,mainly distributed in internal medicine and urology,and antibiotics should be used reasonably in combina
