摘　　要：目的 探讨特定电磁波治疗器(TDP)局部照射对儿童肛周脓肿术后创面愈合的影响。方法 选取2019年6月到2022年7月于浙江省台州医院儿外科住院的儿童肛周脓肿术后患者92例,随机分为试验组与对照组,各46例。两组均行肛周脓肿切开引流术治疗,术后常规抗感染治疗。对照组术后予以常规换药治疗,试验组联合TDP局部照射治疗。比较两组创面愈合时间、感染控制时间和换药总次数、术后创面肉芽生长情况及肛瘘发生率。结果 试验组创面愈合时间、感染控制时间均短于对照组,换药总次数少于对照组,差异均有统计学意义(P <0.05);术后5 d和10 d,试验组创面肉芽生长评分均高于对照组,差异有统计学意义(P <0.05);术后两组分别发生肛瘘2例(4.35%)和8例(17.39%),试验组肛瘘发生率低于对照组,差异有统计学意义(χ~2=4.040,P<0.05)。结论 TDP局部照射用于儿童肛周脓肿术后较常规换药治疗更具优势,不仅可缩短感染控制时间,促进创面肉芽组织生长,加快创面愈合,还可降低肛瘘发生率,改善预后。Objective The effect of specific electromagnetic wave therapeutic device(TDP)partial irradiation on wound healing after perianal abscess surgery in children was investigated.Methods 92 children with perianal abscess who were hospitalized in the pediatric surgery department of Taizhou Hospital in Zhejiang Province from June 2019 to July 2022 were randomly divided into test group and control group,with 46 cases each.Both groups of children were treated with incision and drainage of perianal abscess,and routine antiinfection treatment was performed after operation.After surgery,the children in the control group were treated with routine dressing change,and the children in the test group were treated with TDP partial irradiation.The wound healing time,infection control time,total dressing change times,wound granulation growth and anal fistula incidence were compared between the two groups.Results The wound healing time,infection control time and total dressing change times of children in the test group were shorter than or less than those in the control group(P<0.05);On the 5th and 10th day after operation,the score of granulation growth on the wound of children in the test group was higher than that in the control group(P<0.05);Postoperative anal fistula occurred in 2 cases(4.35%)and 8 cases(17.39%)in the two groups.The incidence of anal fistula in the test group was lower than that in the control group(χ2=4.040,P<0.05).Conclusions The application of TDP partial irradiation to children with perianal abscess after operation has more advantages than conventional dressing change treatment.It can not only shorten the control time of infection,promote the growth of granulation tissue on the wound,accelerate the wound healing,but also reduce the incidence of anal fistula and improve the prognosis.
