针灸治疗食道癌的选穴规律

摘　　要：目的:运用数据挖掘方法探讨针灸治疗食道癌(EC)的取穴规律。方法:检索中国知网数据库、万方学术期刊全文数据库、维普中文科技期刊数据库、中国生物医学文献服务系统中文数据库中针灸治疗EC的文献,筛选文献后提取腧穴处方建立数据库。通过SPSS 26.0、SPSS Modeler 14.1、Clementine 12.0进行聚类分析、关联规则分析、复杂网络分析。结果:共筛选文献68篇,统计腧穴处方68首,涉及4 701例患者,腧穴126个。其中使用频数≥10的腧穴16个。频数前5的腧穴为足三里、中脘、内关、膈俞、三阴交;临床腧穴归经以任脉、胃经、膀胱经为主;最常选用的特定穴为交会穴、募穴、五输穴;腧穴主要分布在四肢及胸腹部;关联分析得出12条2个腧穴之间的关联;聚类分析显示共有7组腧穴。结论:国内医家治疗EC多选用的核心针灸处方为“足三里、中脘、内关”,核心穴对为“足三里-中脘”。针刺方案多选用阴、阳经并重,并注重特定穴使用,尤其是交会穴多选取肝经、脾经相交的腧穴相互搭配。Objective:To explore the rule of acupoint selection in the treatment of esophageal cancer(EC)by data mining.Methods:The literature on acupuncture and moxibustion treatment of EC in CNKI database,Wanfang Academic Journal Full-text Database,VIP Chinese Science and Technology Journal Database,and Chinese Biomedical Literature Service System Chinese Database were searched.After screening the literature,acupoint prescriptions were extracted to establish a database.Cluster analysis,association rule analysis and complex network analysis were carried out by SPSS 26.0,SPSS Modeler 14.1 and Clementine 12.0.Results:A total of 68 literatures were screened,and 68 acupoints prescriptions were counted,involving 4701 cases and 126 acupoints.Among them,16 acupoints with frequency≥10 were used.The first 5 acupoints of frequency are Zusanli(ST36),Zhongwan(RN12),Neiguan(PC6),Geshu(BL17),Sanyinjiao(SP6);the clinical acupoints are mainly Ren meridian,stomach meridian and bladder meridian;the most commonly selected specific points are intersection points,front-mu points and five-shu points;acupoints are mainly distributed in the limbs and chest and abdomen;correlation analysis yielded 12 binomial correlations.Cluster analysis showed that there were 7 groups of acupoints.Conclusion:The core acupuncture prescriptions used by domestic doctors to treat EC are"Zusanli(ST36),Zhongwan(RN12),Neiguan(PC6)",and the core acupoint pair is"Zusanli(ST36)-Zhongwan(RN12)".The acupuncture scheme mostly selects both yin and yang meridians,and pays attention to the use of specific acupoints,especially the intersection of liver and spleen meridians.
