摘  要:【目的】探讨CXC趋化因子配体10(CXCL10)、成纤维细胞生长因子21(FGF-21)、25羟基维生素D[25-(OH)D]预测急性心肌梗死患者经皮冠状动脉介入治疗(PCI)术后不良心血管事件发生风险的价值。【方法】纳入2019年3月至2020年12月宝鸡市人民医院收治的86例行PCI治疗的AMI患者为研究对象,患者经治疗后均获有效随访1年,依据患者随访期内是否发生不良心血管事件分为不良心血管事件组(n=32)和非不良心血管事件组(n=54)。比较两组CXCL10、FGF-21、25-(OH)D水平,采用受试者工作特征曲线(ROC曲线)分析CXCL10、FGF-21、25-(OH)D水平对AMI患者PCI术后不良心血管事件发生风险的预测价值,采用多因素Logistic回归分析CXCL10、FGF-21、25-(OH)D是否为AMI患者PCI术后不良心血管事件发生的危险因素。【结果】观察组CXCL10、FGF-21水平高于对照组(P<0.05),25-(OH)D水平低于对照组(P<0.05)。ROC曲线分析显示,CXCL10、FGF-21、25-(OH)D预测AMI患者PCI术后发生不良心血管事件的曲线下面积分别为0.822、0.716、0.764(均P<0.05)。Logistic回归分析显示,CXCL10、FGF-21、25-(OH)D是AMI患者PCI术后发生不良心血管事件的独立影响因素(P<0.05)。【结论】CXCL10、FGF-21、25-(OH)D预测AMI患者PCI后不良心血管事件发生风险具有重要的临床价值。【Objective】To explore the value of CXC chemokine ligand 10(CXCL10),fibroblast growth factor 21(FGF-21),and 25-hydroxyvitamin D[(25-OH)D]in predicting the risk of adverse cardiovascular events in patients with acute myocardial infarction after percutaneous coronary intervention(PCI).【Methods】A total of 86 patients with AMI treated with PCI from March 2019 to December 2020 were included in the study.After treatment,the patients were effectively followed up for 1 year.They were divided into adverse cardiovascular events group(n=32)and non conscientious vascular events group(n=54)based on whether they had adverse cardiovascular events during the follow-up period.The levels of CXCL10,FGF-21,and 25-(OH)D were compared between the two groups.The predictive value of CXCL10,FGF-21,and 25-(OH)D levels on the risk of adverse cardiovascular events in patients with AMI after PCI was analyzed using the receiver operational characteristic curve(ROC curve).Multivariate logistic regression was used to analyze whether CXCL10,FGF-21,and 25-(OH)D were risk factors for adverse cardiovascular events in patients with AMI after PCI.【Results】The levels of CXCL10 and FGF-21 in the observation group were higher than those in the control group(P<0.05),and the levels of 25-(OH)D were lower than those in the control group(P<0.05).ROC curve analysis showed that the area under the curve of CXCL10,FGF-21,and 25-(OH)D predicting adverse cardiovascular events in patients with AMI after PCI was 0.822,0.716,and 0.764,respectively(all P<0.05).Logistic regression analysis showed that CXCL10,FGF-21,and 25-(OH)D were independent influencing factors for adverse cardiovascular events in patients with AMI after PCI(P<0.05).【Conclusion】CXCL10,FGF-21,and 25-(OH)D have important clinical value in predicting the risk of adverse cardiovascular events in patients with AMI after PCI.
