彩色多普勒超声在急性输尿管结石临床诊断中诊断敏感度及阳性预测值的影响分析

摘　　要：目的:分析彩色多普勒超声在急性输尿管结石临床诊断中对诊断敏感度及阳性预测值的影响。方法:将2022年1月—2022年12月接收的110例疑似急性输尿管结石患者纳入研究,均实施灰阶超声和彩色多普勒超声检查,若无法查出,再行CT检查,最后均进行体外超声波碎石治疗,把治疗后经尿液排出结石作为诊断金标准,观察两种检查方式的诊断效能指标,包括诊断准确率、敏感度、特异度、阳性预测值、阴性预测值,同时评估患者对不同检查方式的满意指数及检查前后的焦虑程度。结果:治疗后显示,110例疑似急性输尿管结石含阴性21例,阳性89例;彩色多普勒超声检查的诊断准确率、敏感度、特异度、阳性预测值、阴性预测值均显著高于灰阶超声检查(P<0.05);患者对彩色多普勒超声检查的满意指数高于灰阶超声检查(P<0.05);检查前,拟接受彩色多普勒超声和灰阶超声检查患者的焦虑评估分数无统计学意义(P>0.05),检查后,接受彩色多普勒超声检查患者的焦虑评估分数低于灰阶超声检查(P<0.05)。结论:彩色多普勒超声在急性输尿管结石临床诊断中有较高价值,不仅提升诊断准确率、敏感度、阳性预测值、阴性预测值,并增加满意指数和改善就诊时的焦虑情绪。Objective:To analyze the influence of color Doppler ultrasound on the diagnostic sensitivity and positive predictive value in the clinical diagnosis of acute ureteral calculi.Methods:110 patients with suspected acute ureteral calculi received from January 2022 to December 2022 were included in the study.They were all examined by gray-scale ultrasound and color Doppler ultrasound.If they could not be detected,they were examined by CT.Finally,they were treated by extracorporeal ultrasound lithotripsy.After treatment,the excretion of stones in urine was used as the diagnostic gold standard.The diagnostic efficacy indicators of the two methods were observed, including diagnostic accuracy, sensitivity, specificity, positive predictive value Negative predictive value, meanwhile, the satisfaction index of patients with different examination methods and the degree of anxiety before and after examination were evaluated.Results: Display after treatment, 21 cases were negative and 89 cases were positive in 110 suspected acute ureteral calculi;The diagnostic accuracy, sensitivity, specificity, positive predictive value and negative predictive value of color Doppler ultrasound were significantly higher than those of gray scale ultrasound ( P <0.05);The satisfaction index of patients with color Doppler ultrasound was higher than that with gray scale ultrasound ( P <0.05);Before the examination, the anxiety assessment score of the patients who were to receive color Doppler ultrasound and gray scale ultrasound was not statistically significant ( P >0.05). After the examination, the anxiety assessment score of the patients who received color Doppler ultrasound was lower than that of gray scale ultrasound ( P <0.05).Conclusion: Color Doppler ultrasound has a high value in the clinical diagnosis of acute ureteral calculi, which not only improves the diagnostic accuracy, sensitivity, positive predictive value, negative predictive value, but also increases the satisfaction index and improves the anxiety at the time of treatment.
