锁骨骨折锁定钢板内固定术后再骨折的因素分析及治疗探讨

摘　　要：目的探讨造成锁骨骨折锁定钢板内固定术后再骨折的发生因素及相关治疗。方法选取2012年6月至2020年6月本院锁骨骨折锁定钢板内固定术后再骨折患者31例,依据治疗方法分为手术内固定治疗组(手术治疗组,n=20)、非手术治疗组(n=11)两组,统计分析两组患者的疼痛程度、骨折愈合时间、肩关节功能、临床疗效。结果手术治疗组患者治疗后较治疗前的VAS评分降低幅度显著高于非手术治疗组(P<0.05),骨折愈合时间显著短于非手术治疗组(P<0.05)。手术治疗组患者治疗后较治疗前的疼痛程度、功能活动、肩关节活动度、肌力评分及总分升高幅度均显著高于非手术治疗组(P<0.05)。手术治疗组患者的总有效率为95.0%(19/20),显著高于非手术治疗组81.8%(9/11)(P<0.05)。结论锁骨骨折锁定钢板内固定术后再骨折的手术治疗效果优于非手术治疗。Objective To investigate the treatment of refracture after locking plate internal fixation for clavicle fracture.Methods A retrospective selection in June 2012 to June 2020 in our hospital locking plate fixation for clavicle fracture was performed.31 cases of patients with postoperative treatment were divided into the surgical internal fixation treatment group(surgical treatment group,n=20)and the non-surgical treatment group(n=11).In the two groups,the patients'level of pain,fracture healing time,shoulder joint function,clinical curative effect were analyzed.Results Visual analogue scale(VAS)score in the surgical treatment group was significantly lower than that before the treatment(P<0.05),and fracture healing time was significantly shorter than that in the non-surgical treatment group(P<0.05).The postoperative pain,functional activity,shoulder range of motion,muscle strength score and total score in the surgical treatment group were significantly higher than those in the non-surgical treatment group(P<0.05).The total effective rate of 95.0%(19/20)in the surgical treatment group was significantly higher than that of 81.8%(9/11)in the non-surgical treatment group(P<0.05).Conclusion The curative effect of refracture after locking plate internal fixation for clavicle fracture is better than that of non-surgical treatment.
