摘  要:目的探讨3D-Slicer软件辅助神经内镜微创手术对高血压性脑出血患者神经功能预后的影响。方法分析2018年4月至2021年10月于本院神经外科及ICU科收治的行手术治疗的60例高血压性脑出血(基底节区出血)患者临床资料,根据手术方式的不同,分为观察组(n=30)和对照组(n=30),观察组行3D-Slicer软件辅助神经内镜下微创手术治疗,对照组采用传统CT定位开颅显微镜下血肿清除术。对比分析两组患者的临床资料。结果术后24 h血肿清除率观察组优于对照组(t=10.729,P<0.01),术后7 d血肿清除率观察组优于对照组(t=9.973,P<0.01)。术后再出血率观察组低于对照组(P=0.024)。手术时间观察组低于对照组(t=-10.909,P<0.01)。住院天数观察组少于对照组(t=-6.971,P<0.01)。术后颅内感染、脑梗塞、切口愈合不良发生率,观察组低于对照,但无统计学意义(P>0.05)。术后肺部感染、深静脉血栓的发生率,观察组低于对照组(P<0.05)。术后6个月GOS评分,观察组高于对照组(Z=-2.437,P<0.05)。结论3D-Slicer软件辅助神经内镜微创手术治疗高血压性脑出血(基底节区出血),手术创伤小,血肿清除率高,再出血率低,手术时间短,并发症少,能改善患者神经功能预后。Objective To investigate the effects of 3D-Slicer software assisted neuroendoscopic minimally invasive surgery on the neurological prognosis of patients with hypertensive intracerebral hemorrhage.Methods Clinical data of 60 patients with hypertensive intracerebral hemorrhage(Basal ganglia hemorrhage)were retrospectively analyzed.According to different surgical methods,they were divided into the observation group(n=30)and the control group(n=30).The observation group was treated with 3D-Slicer software assisted neuroendoscopic minimally invasive surgery,and the control group was treated with traditional CT localization craniotomy and microscopic hematoma removal.The clinical data of the two groups were compared and analyzed.Results The hematoma clearance rate of the observation group was better than that of the control group 24 hours after operation,with statistical significance(t=10.729,P<0.01),and that of the observation group 7 days after operation was better than that of the control group,with statistical significance(t=9.973,P<0.01).The postoperative rebleeding rate in the observation group was lower than that in the control group,with statistical significance(P=0.024).The operation time of observation group was lower than that of control group,with statistical significance(t=-10.909,P<0.01).The days of hospitalization in the observation group were less than that in the control group,with statistical significance(t=-6.971,P<0.01).The incidence of postoperative intracranial infection,cerebral infarction and poor wound healing was lower in the observation group than in the control group,but there was no statistical significance(P>0.05).The incidence of postoperative pulmonary infection and deep vein thrombosis in the observation group was lower than that in the control group,with statistical significance(P<0.05).The GOS score 6 months after operation was higher in the observation group than in the control group,with statistical significance(Z=-2.437,P<0.05).Conclusion 3D-Slicer software assisted neuroendoscopic
