糖尿病合并视网膜病变的发病率及相关因素分析

摘　　要：目的:研究本地区糖尿病(DM)患者糖尿病性视网膜病变(DR)的发生情况及相关危险因素。方法:选取2018年1月至2021年12月期间就诊于通化市中心医院的患有DM的通化地区居民共计3237例,根据眼底检查结果分为患有DR的DR组(695例)和无DR的NDR组(2542例)。记录两组患者的DM病程﹑视力、空腹血糖﹑收缩压等临床资料,并进行统计学分析。结果:3237例DM患者DR的发病率为21.47%。其中增生型DR(PDR)的发病率为2.84%;单纯型DR(NPDR)的发病率为18.63%,轻度、中度和重度NPDR的发病率分别为5.75%、8.12%和4.76%。不同性别患者DR的发病率相比,无统计学差异(P>0.05)。不同年龄、DM病程患者DR的发病率相比,具有统计学差异(P<0.05)。DR组的年龄大于NDR组,DM病程长于NDR组,收缩压、空腹血糖均高于NDR组,差异有统计学意义(P<0.05)。结论:DR在通化地区DM患者中的发病率为21.47%,高龄、长DM病程、高空腹血糖以及收缩压增高为DR的危险因素。Objective:To study the incidence and related risk factors of diabetic retinopathy(DR)in patients with diabetes mellitus(DM)in this region.Methods:A total of 3237 Tonghua residents with DM were selected from Tonghua Central Hospital from January 2018 to December 2021.According to the results of fundus examination,they were divided into DR Group(695 cases)with DR And NDR gro up(2542 cases)without DR.The course of DM,visual acuity,fasting blood glucose,systolic blood pressure and other clinical data of the two groups were recorded,and statistically analyzed.Results:The incidence of DR In 3237 DM patients was 21.47%.The incidence of proliferative DR(PDR)was 2.84%.The incidence of simple type DR(NPDR)was 18.63%,and the incidence of mild,moderate and severe NPDR was 5.75%,8.12%and 4.76%,respectively.There was no significant difference in the incidence of DR Among different genders(P>0.05).There were statistical differences in the incidence of DR Among DM patients of different ages and course(P<0.05).The age of DR Group was longer than that of NDR group,the course of DM was longer than that of NDR group,systolic blood pressure and fasting blood glucose were higher than those of NDR group,the difference was statistically significant(P<0.05).Conclusion:The incidence rate of DR In DM patients in Tonghua area is 21.47%.Old age,long course of DM,high fasting blood glucose and increased systolic blood pressure are risk factors for DR.
