加味桃红四物汤联合PFNA对股骨粗隆间骨折患者疼痛及髋关节功能的影响

摘　　要：目的观察加味桃红四物汤联合股骨近端防旋髓内钉(PFNA)术对股骨粗隆间骨折(IFF)患者疼痛及髋关节功能的影响。方法选取2020年3月至2022年1月南昌市洪都中医院收治的78例IFF患者,通过随机数字表法分为两组,对照组、观察组各39例,对照组接受PFNA术治疗,观察组接受PFNA术联合加味桃红四物汤治疗,比较两组肿胀消退及骨折愈合时间。于术前、术后1周、术后2周,比较两组疼痛程度[视觉模拟评分(VAS)];于术前、术后1个月、术后3个月,比较两组髋关节功能[髋关节评分量表(Harris)];比较并发症发生率。结果与对照组比较,观察组术后恢复时间(肿胀消退及骨折愈合)均较短,且并发症发生率更低(P<0.05);术后1周、2周,两组VAS评分较术前均下降,且观察组各时点VAS评分低于对照组;两组患者术后Harris髋关节功能评分不断提高,且观察组升高更为显著,差异有统计学意义(P<0.05)。结论加味桃红四物汤联合PFNA术应用于IFF患者中,可减轻患者术后疼痛感,促进骨折愈合,提高髋关节功能,且并发症少。Objective To observe the effect of modified taohong siwu decoction combined with proximal femoral anti-rotation intramedullary nail (PFNA) on pain and hip function in patients with intertrochanteric femoral fracture (IFF). Methods A total of 78 patients with IFF who were admitted to Nanchang Hongdu Hospital of Traditional Chinese Medicine from March 2020 to January 2022 were selected and divided into two groups by random number table method, 39 cases in the control group and 39 cases in the observation group. The control group received PFNA treatment, the observation group received PFNA combined with modified taohong siwu decoction, and the swelling subsidence and fracture healing time were compared between the two groups. The pain degree of the two groups was compared before operation, 1 week after operation and 2 weeks after operation [VAS];the hip joint function of the two groups was compared before operation, 1 month after operation and 3 months after operation (Harris);the incidence of complications was compared. Results Compared with the control group, the postoperative recovery time (swelling subsidence and fracture healing) of the observation group were shorter, and the incidence of complications was lower (P<0.05). At 1 week and 2 weeks after operation, the VAS score of the two groups decreased compared with that before operation, and the VAS score of the observation group was lower than that of the control group at each time point;Harris hip joint function score of patients in the two groups improved continuously after operation, and the increase in the observation group was more significant, with statistically significant difference (P<0.05). Conclusion Modified taohong siwu decoction combined with PFNA in patients with IFF can reduce postoperative pain, promote fracture healing, and improve hip function with fewer complications.
