炔雌醇环丙孕酮联合米非司酮治疗卵巢巧克力囊肿的临床观察

摘　　要：目的分析卵巢巧克力囊肿疾病特点,评价炔雌醇环丙孕酮与米非司酮联合治疗的预后效果。方法80例卵巢巧克力囊肿患者,依据随机法分为观察组与对照组,各40例。对照组采用腹腔镜手术治疗,观察组在对照组基础上采用炔雌醇环丙孕酮联合米非司酮治疗。对比两组治疗后1周的临床疗效,临床症状改善情况及妊娠率,治疗前后雌激素[卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E2)、孕酮(P)]水平。结果观察组治疗总有效率97.50%高于对照组的75.00%,差异有统计学意义(P<0.05)。观察组痛经改善率、性交痛改善率、腰骶坠胀改善率、卵巢储备功能恢复正常率、妊娠率均高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组P(1.51±0.25)nmol/L、E2(40.55±6.80)pmol/L、LH(17.23±3.20)IU/L、FSH(29.95±5.05)mU/L均优于对照组的(3.05±0.50)nmol/L、(63.30±8.80)pmol/L、(13.40±3.05)IU/L、(17.89±2.30)mU/L,差异均有统计学意义(P<0.05)。结论腹腔镜术后采用炔雌醇环丙孕酮联合米非司酮治疗卵巢巧克力囊肿有助于改善卵巢储备功能,进而提升预后效果。Objective To analyze the characteristics of ovarian chocolate cyst disease and evaluate the prognostic effect of combined treatment of ethinylestradiol and cyproterone acetate and mifepristone.Methods A total of 80 patients with ovarian chocolate cysts were randomly divided into observation group and control group,with 40 cases in each group.The control group was treated with laparoscopic surgery,and the observation group was treated with ethinylestradiol and cyproterone acetate in combination with mifepristone on the basis of the control group.Both groups were compared in terms of clinical efficacy,improvement of clinical symptoms,pregnancy rate,estrogen[follicle forming hormone(FSH),luteinizing hormone(LH),estradiol(E2)and progesterone(P)]levels before and after treatment 1 week after treatment.Results The total effective rate was 97.50%in the observation group,which was higher than that of 75.00%in the control group,and the difference was statistically significant(P<0.05).The improvement rates of dysmenorrhea,coital pain,lumbosacral distension,recovery rate of ovarian reserve and pregnancy rate in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the observation group had P of(1.51±0.25)nmol/L,E2 of(40.55±6.80)pmol/L,LH of(17.23±3.20)IU/L,and FSH of(29.95±5.05)mU/L,which were better than those of(3.05±0.50)nmol/L and(63.30±8.80)pmol/L,(13.40±3.05)IU/L,and(17.89±2.30)mU/L in the control group,and the differences were statistically significant(P<0.05).Conclusion Ethinylestradiol and cyproterone acetate combined with mifepristone in the treatment of ovarian chocolate cysts after laparascopic surgery can help improve ovarian reserve and improve the prognosis.
