摘  要:目的探讨宣肺通窍汤对分泌性中耳炎(SOM)大鼠的作用及与TLR4/MyD88/NF-κB通路相关机制。方法从50只大鼠中随机选取10只为对照组,其余大鼠建立SOM模型,建模过程中4只大鼠建模失败,其余大鼠随机分为4组,各9只,分别为SOM组,宣肺通窍汤低、高剂量组及头孢克洛组。宣肺通窍汤低、高剂量组给予0.5、1.5 g·mL^(-1)浓度的宣肺通窍汤,10 mL·kg^(-1)·d^(-1),头孢克洛组给予头孢克洛0.1 g·kg^(-1)·d^(-1),对照组与SOM组灌胃给予等量的生理盐水,连续灌胃给药7 d。药物干预后,检测大鼠耳腔灌洗液IL-4、IFN-γ水平,测量大鼠中耳黏膜厚度及中性粒细胞数量,观察大鼠中耳黏膜病理学变化,并检测鼓膜组织TLR4/MyD88/NF-κB通路蛋白表达情况。结果与对照组比较,SOM组IL-4水平升高、IFN-γ水平降低,中耳黏膜厚度及中性粒细胞计数增加,TLR4、MyD88及p-NF-κB/NF-κB表达上调(P<0.05);与SOM组比较,宣肺通窍汤低、高剂量组及头孢克洛组IL-4水平降低、IFN-γ水平升高,中耳黏膜厚度及中性粒细胞计数减少,TLR4、My D88及p-NF-κB/NF-κB表达下调(P<0.05);与宣肺通窍汤低剂量组比较,宣肺通窍汤高剂量组及头孢克洛组IL-4水平降低、IFN-γ水平升高,中耳黏膜厚度及中性粒细胞计数减少,TLR4、My D88及p-NF-κB/NF-κB表达下调(P<0.05);与宣肺通窍汤高剂量组比较,头孢克洛组IL-4水平降低、IFN-γ水平升高,中耳黏膜厚度及中性粒细胞计数减少,TLR4、MyD88及p-NF-κB/NF-κB表达下调(P<0.05)。结论宣肺通窍汤能够改善分泌性中耳炎大鼠中耳黏膜厚度及中性粒细胞数量,降低炎症因子水平,其作用与抑制TLR4/MyD88/NF-κB通路有关。Objective To investigate the effect of Xuanfei Tongqiao Decoction on secretory otitis media(SOM)in rats and the related mechanism of TLR4/MyD88/NF-κB pathway.Methods Ten from the 50 rats were randomly selected as the control group,and the remaining rats established SOM models,and 4 rats failed in the modeling process.The remaining rats were randomly divided into SOM group,Xuanfei Tongqiao Decoction low-dose group,high-dose group and cefaclor group separately,9 rats in each group.Xuanfei Tongqiao Decoction low-dose and high-dose groups were given Xuanfei Tongqiao Decoction at concentrations of 0.5g·g·mL^(-1)and 1.5g·mL^(-1),10 mL·kg^(-1)·d^(-1);cefaclor group was given cefaclor 0.1g·kg^(-1)·d^(-1),the control group and SOM group were given the same amount of normal saline by gavage,continuous gavage for 7 days.After drug intervention,the levels of interleukin-4(IL-4)and interferon-γ(IFN-γ)in the lavage fluid of the ear cavity were detected,and the thickness of middle ear mucosa and the number of neutrophils were measured.The pathological changes of middle ear mucosa and the expression of TLR4/MyD88/NF-κB pathway in tympanic membrane were observed.Results Compared with the control group,the level of IL-4 was increased,the level of IFN-γwas decreased,the thickness of middle ear mucosa and neutrophil count were increased,and the expression of TLR4,MyD88 and p-NF-κB/NF-κB was up-regulated in SOM group(P<0.05).Compared with SOM group,the levels of IL-4,middle ear mucosal thickness and neutrophil count were decreased,the level of IFN-γwas increased,and the expression of TLR4,MyD88 and P-NF-κB/NF-κB was down-regulated in Xuanfei Tongqiao decoction low-dose and high-dose groups and cefaclor group(P<0.05).Compared with the Xuanfei Tongqiao Decoction low-dose group,the levels of IL-4,middle ear mucosal thickness and neutrophil count were decreased,the level of IFN-γwas increased and the expression of TLR4,MyD88 and p-NF-κB/NF-κB was down-regulated in the Xuanfei Tongqiao decoction high-dose group and cef
