摘 要:目的 通过临床对比研究证明弹性固定对治疗髁间嵴撕脱骨折的优良性。方法 通过研究保定市第一医院2018年1月至2020年12月40例Meyers-Mckeever Ⅲ型髁间嵴撕脱骨折病例资料,分别采用关节镜下空心钉固定(20例,空心钉固定组)和关节镜下弹性固定(20例,弹性固定组)。对手术时间、骨折愈合时间,术后3、6个月的膝关节Lysholm评分、数字分级量表(NRS)疼痛评分及日常生活能力(ADL)评分指标进行统计。结果 术后随访12个月。弹性固定组的手术时间,术后3、6个月的Lysholm评分、NRS疼痛评分及ADL评分优于空心钉固定组,差异有统计学意义(P<0.05)。两组的骨折愈合时间比较,差异无统计学意义(P>0.05)。结论 关节镜下弹性固定治疗髁间嵴撕脱骨折具有手术时间短、术后功能好、痛苦小、避免二次取出的优点。Objective To demonstrate the superiority of elastic fixation in the treatment of intercondylar crest avulsion fractures through clinical comparative studies.Methods From January 2018 to December 2020,40 cases of Meyers-Mckeever typeⅢintercondylar crest avulsion fractures in the First Hospital of Baoding City were studied and divided into two groups,which were fixed by arthroscopic cannulated screw(20 cases,cannulated screw fixation group)and arthroscopic elastic fixation(20 cases,elastic fixation group).The operation time,fracture healing time,knee Lysholm score,digital grading scale(NRS)pain score,and the daily living ability(ADL)score at 3 and 6 months after the surgery were counted.Results Patients were followed-up after surgery for 12 months.The operation time,the Lysholm score,NRS pain score,and the ADL score in elastic fixation group were better than those in cannulated screw fixation group at 3 and 6 months after the surgery,and the differences between the two groups were statistically significant(P<0.05).The fracture healing time of the two groups was similar,and the difference was not statistically significant(P>0.05).Conclusion Arthroscopic elastic fixation in the treatment of intercondylar crest avulsion fractures has the advantages of short surgical time,good postoperative function,small pain and avoidance of secondary removal.
