摘 要:目的 探讨儿童食管正常超声声像图特征及低高频超声联合检测在腐蚀性食管炎诊治中的应用价值。方法 选取2018年7月—2019年2月在厦门市儿童医院行彩超检查的健康儿童200例和腐蚀性食管炎患儿13例为研究对象,年龄均≤10周岁,利用高频和低频超声对食管进行整体观察并测量前后径、左右径、周长及面积,采用Spearman相关性分析探讨年龄与食管径线的关系,并与腐蚀性食管炎进行比较。结果 儿童食管颈段超声显示率为100.00%,腹段超声显著率为100.0%,胸段超声显著率为95.5%。食管颈段、胸段及腹段各径线与年龄均呈正相关关系,其中以颈段相关性最强(前后径:r=0.423,P<0.05;左右径:r=0.683,P<0.05;周长:r=0.703,P<0.05;面积:r=0.654,P<0.05)。腐蚀性食管炎胸段径线测量结果相较于正常组食管壁明显增厚,差异有统计学意义(均P<0.05)。结论 低高频超声联合可用于儿童食管检查,且在腐蚀性食管炎诊治具且有较高的诊断价值。Objective To explore the characteristics of normal esophageal ultrasonography in children and the application value of combined low and high frequency ultrasonography in the diagnosis and treatment of corrosive esophagitis.Methods A total of 200 healthy children and 13 children with corrosive esophagitis who underwent color ultrasound examination in Xiamen Children’s Hospital from July 2018 to February 2019 were selected as research objects, all of whom were less than or equal to 10 years old.The posterior diameter, left and right diameter, circumference and area of the esophagus were measured by high-frequency and low-frequency ultrasound.Spearman correlation analysis was used to explore the relationship between age and esophageal diameter, and compared with corrosive esophagitis.Results The rates of pediatric esophageal demonstration by ultrasonography were 100.0%(cervical part),100.0%(abdominal part),95.5%(thoracic part).The diameters of the cervical, thoracic and abdominal segments of the esophagus were positively correlated with age, especially the cervical part(width: r=0.423,P<0.05,length: r=0.683,P<0.05,circumference: r=0.703,P<0.05,area: r=0.654,P<0.05).The thoracic part diameter of corrosive esophagitis was statistically significant, compared to the normal group of esophagus(all P<0.05).Conclusion High frequency and low frequency ultrasound can be used for pediatric esophageal examination, and has a high application value in the diagnosis and treatment of corrosive esophagitis.
