卡维地洛联合美托洛尔对老年高血压伴心律失常患者心脏结构及血清hs-CRP水平的影响

摘　　要：目的:分析卡维地洛+美托洛尔对老年高血压伴心律失常患者的具体影响。方法:选取2020年4月-2022年4月高安市中医院诊治的76例老年高血压伴心律失常患者,按随机数字表法分为两组,各38例。对照组给予美托洛尔治疗,观察组加以卡维地洛治疗,连续用药3个月。对比两组临床疗效、心脏结构、血压、超敏C反应蛋白(hs-CRP)、不良反应发生情况。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗前,两组左室总量指数(LVMI)、左室球径指数(LVGI)、左房内径(LAD)、左心室射血分数(LVEF)、收缩压(SBP)、舒张压(DBP)、hs-CRP相比,差异均无统计学意义(P>0.05);治疗后,观察组LVMI、LVGI、LAD、SBP、DBP、hs-CRP均低于对照组,LVEF高于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率相当,差异无统计学意义(P>0.05)。结论:卡维地洛联合美托洛尔治疗老年高血压伴心律失常患者效果显著,可有效改善其心脏结构,降低血压与hs-CRP水平,且无严重不良反应,值得临床的大力推广。Objective:To analyze the specific effects of Carvedilol+Metoprolol on elderly hypertensive patients with arrhythmia.Method:A total of 76 elderly patients with hypertension and arrhythmia diagnosed and treated in Gao'an Hospital of Traditional Chinese Medicine from April 2020 to April 2022 were selected and divided into two groups according to the random number table method,38 cases in each group.The control group was treated with Metoprolol,and the observation group was treated with Carvedilol on the basis of the control group for 3 months.The clinical efficacy,cardiac structure,blood pressure,high-sensitivity C reactive protein(hs-CRP)and adverse reactions of the two groups were compared.Result:The total effective rate of the observation group was higher than that of the control group(P<0.05);before treatment,there were no significant differences in left ventricular mass index(LVMI),left ventricular global-diameter index(LVGI),left atrial diameter(LAD),left ventricular ejection fraction(LVEF),systolic blood pressure(SBP),diastolic blood pressure(DBP)and hs-CRP between the two groups(P>0.05);after treatment,LVMI,LVGI,LAD,SBP,DBP,hs-CRP in the observation group were lower than those in the control group,and LVEF was higher than that in the control group(P<0.05);the incidence of adverse reactions in the two groups was similar,the difference was not statistically significant(P>0.05).Conclusion:Carvedilol combined with Metoprolol in the treatment of elderly patients with hypertension and arrhythmia has a significant effect,which can effectively improve their cardiac structure,reduce blood pressure and hs-CRP levels,and has no serious adverse reactions.It is worthy of clinical promotion.
