慢性荨麻疹患者外周血IL-17、IL-25和IL-33的表达水平及诊断价值研究

摘　　要：目的:分析慢性荨麻疹患者外周血白细胞介素-17(IL-17)、白细胞介素-25(IL-25)和白细胞介素-33(IL-33)的表达水平,探讨其对慢性荨麻疹患者的诊断价值。方法:选取2020年1月-2021年12月张家港市第一人民医院(苏州大学附属张家港医院)皮肤科收治的慢性荨麻疹患者108例作为病例组,根据病情严重程度将病例组分为轻度组(38例)、中度组(33例)、重度组(37例),另选取同期体检的健康志愿者42例作为对照组。比较病例组与对照组外周血IL-17、IL-25和IL-33水平,比较轻、中、重度三组患者外周血IL-17、IL-25、IL-33水平及症状评分。采用受试者工作特征(ROC)曲线分析IL-17、IL-25、IL-33预测病情严重程度的效能。结果:病例组患者外周血IL-17、IL-25、IL-33水平均高于对照组(P<0.05);与轻度组相比,中、重度组患者外周血IL-17、IL-25、IL-33水平均明显较高(P<0.05),与中度组相比,重度组患者外周血IL-17、IL-25、IL-33水平均明显较高(P<0.05);慢性荨麻疹患者症状评分与外周血IL-17、IL-25、IL-33水平均呈正相关(P<0.05);ROC曲线分析显示,外周血IL-17、IL-25、IL-33反映慢性荨麻疹患者轻度与中度病情严重程度的ROC曲线下面积分别为0.789、0.841、0.786,反映慢性荨麻疹患者中度与重度病情严重程度的ROC曲线下面积分别为0.809、0.853、0.813。结论:慢性荨麻疹患者外周血IL-17、IL-25和IL-33的表达水平均升高,且均与患者病情严重程度有关,可作为判断慢性荨麻疹发生发展的辅助指标。Objective:To analyze the expression levels of IL-17,IL-25 and IL-33 in peripheral blood of patients with chronic urticaria,and explore their diagnostic value in patients with chronic urticaria.Method:A total of 108 patients with chronic urticaria admitted to the Dermatology Department of Zhangjiagang First People's Hospital(Zhangjiagang Hospital Affiliated to Soochow University)from January 2020 to December 2021 were selected as the case group.According to the severity of the disease,the case group was divided into mild group(38 cases),moderate group(33 cases)and severe group(37 cases).In addition,42 healthy volunteers who underwent physical examination during the same period were selected as the control group.The levels of IL-17,IL-25 and IL-33 in peripheral blood of patients in the case group and the control group were compared.The levels of IL-17,IL-25 and IL-33 and symptom scores in peripheral blood of patients in the mild,moderate and severe groups were compared.The efficacy of IL-17,IL-25 and IL-33 in predicting disease severity were analyzed by receiver operating characteristic(ROC)curve.Result:The levels of IL-17,IL-25 and IL-33 in peripheral blood of the case group were higher than those of the control group(P<0.05).Compared with the mild group,the levels of IL-17,IL-25,IL-33 in peripheral blood of patients in moderate and severe groups were significantly higher(P<0.05);compared with the moderate group,the levels of IL-17,IL-25,IL-33 in peripheral blood of patients in severe group were significantly higher(P<0.05).Symptom scores of patients with chronic urticaria were positively correlated with levels of IL-17,IL-25 and IL-33 in peripheral blood(P<0.05).ROC curve analysis showed that the area under the ROC curve of mild and moderate severity of chronic urticaria patients with IL-17,IL-25 and IL-33 in peripheral blood were 0.789,0.841 and 0.786,respectively.The area under the ROC curve of moderate and severe severity in patients with chronic urticaria were 0.809,0.853 and 0.813,respectively.Conclusion:The
