关节镜监视下内固定术联合加味紫金丹治疗胫骨平台骨折的临床研究

摘　　要：目的:探究关节镜监视下内固定术联合加味紫金丹治疗胫骨平台骨折的临床效果。方法:选取2020年1月-2021年12月于江西省中西医结合医院骨伤科接受治疗的60例胫骨平台骨折患者为研究对象,按照随机数字表法分为两组,各30例。对照组采用关节镜监视下内固定术治疗,研究组在对照组基础上联合加味紫金丹治疗。对比两组围手术期指标、视觉模拟评分法(VAS)评分、美国特种外科医院膝关节评分(HSS)及并发症发生率。结果:研究组住院时间及骨折愈合时间均短于对照组,术中出血量少于对照组,VAS评分低于对照组(P<0.05);两组手术时间对比,差异无统计学意义(P>0.05)。研究组术后3、6个月HSS评分均高于对照组(P<0.05)。研究组并发症发生率为3.33%,低于对照组的26.67%(P<0.05)。结论:胫骨平台骨折患者应用关节镜监视下内固定术联合加味紫金丹治疗能够有效减轻术后疼痛感,促进骨折端愈合和膝关节功能恢复,并发症少,安全性高,值得推广。Objective:To explore the clinical effect of internal fixation under arthroscopic monitoring combined with Jiawei Zijindan in the treatment of fracture of the tibial plateau.Method:A total of 60 patients with fracture of the tibial plateau who were treated in the Department of Orthopedics and Traumatology,Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to December 2021 were selected as the research subjects and divided into two groups according to the random number table method,with 30 cases in each group.The control group was treated with internal fixation under arthroscopic monitoring,and the study group was treated with Jiawei Zijindan on the basis of the control group.The perioperative indicators,visual analogue scale(VAS)scores,hospital for special surgery knee score(HSS)and the incidence of complications were compared between the two groups.Result:The hospital stay and fracture healing time in the study group were shorter than those in the control group,and the intraoperative bleeding volume was less than that in the control group,and the VAS scores was lower than that in the control group(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).The HSS scores of the study group was higher than that of the control group at 3 and 6 months after operation(P<0.05).The incidence of complications in the study group was 3.33%,which was lower than 26.67%in the control group(P<0.05).Conclusion:The application of internal fixation under arthroscopic monitoring combined with Jiawei Zijindan in patients with fracture of the tibial plateau can effectively reduce postoperative pain,promote fracture healing and knee joint function recovery,with few complications and high safety,which is worth popularizing.
