CT影像分析OMC变异与鼻中隔偏曲及鼻旁窦炎的关系

摘　　要：目的探究CT影像分析窦口鼻道复合体(OMC)变异与鼻中隔偏曲及鼻旁窦炎的关系。方法以回顾性分析的方式共纳入我院进行鼻与鼻旁窦CT检查的180例患者作为研究对象,病例选取起始时间2019年2月,截止时间2021年8月。收集患者个人资料以及影像学资料,根据患者是否存在鼻中隔偏曲分为甲组(无鼻中隔偏曲)和乙组(有鼻中隔偏曲),组间数据比较。结果甲组和乙组组间比较OMC变异发生率、单侧以及双侧OMC变异发生率均存在显著差异(P<0.05)。甲组和乙组组间比较鼻旁窦炎发生率、单侧以及双侧鼻旁窦炎发生率比较无显著差异(P>0.05)。A组和B组组间比较鼻旁窦炎发生率无显著差异(P>0.05)。结论CT影像能较好的显示OMC变异情况,判断其与鼻中隔偏曲及鼻旁窦炎的关系,鼻中隔偏曲患者鼻腔增宽侧OMC变异属于补偿机制,而不能确定其为鼻旁窦炎的阻塞原因。Objective To explore the relationship between the variation of ostiomeatal complex(OMC)and deviation of nasal septum and paranasal sinusitis.Methods A total of 180 patients who underwent CT examination of nasal and paranasal sinuses in our hospital were included in the study by retrospective analysis.The cases were selected from February 2019 to August 2021.The personal data and imaging data of the patients were collected.According to whether the patients had nasal septum deviation,they were divided into group A(no nasal septum deviation)and group B(with nasal septum deviation).The data were compared between the groups.Results There were significant differences in the incidence of OMC variation,unilateral and bilateral OMC variation between group A and group B(P<0.05).There was no significant difference in the incidence of paranasal sinusitis,unilateral and bilateral paranasal sinusitis between group A and group B(P>0.05).There was no significant difference in the incidence of paranasal sinusitis between group A and group B(P>0.05).Conclusion CT images can better show the variation of OMC and judge its relationship with nasal septum deviation and paranasal sinusitis.The variation of OMC on the widened side of nasal cavity in patients with nasal septum deviation belongs to the compensation mechanism,but it can not be determined as the cause of obstruction of paranasal sinusitis.
