摘  要:目的探讨妊娠糖尿病(GDM)患者血清胎盘蛋白13(PP13)、胰岛素样生长因子-1(IGF-1)与胎儿生长受限(FGR)的相关性。方法对2020年2月至2021年9月南阳市第一人民医院收治的135例GDM患者进行前瞻性病例对照研究,根据是否发生FGR分为发生组与未发生组。采用χ^(2)检验、独立样本t检验比较两组患者一般资料、实验室指标,采用Pearson相关性检验分析患者孕早期血清PP13、IGF-1水平与新生儿出生体重、头围的相关性,采用logistic回归分析孕早期血清PP13、IGF-1水平对GDM患者发生FGR的影响及二者之间的交互作用。结果135例GDM患者中,发生FGR 39例,发生率为28.89%。发生组新生儿出生体重、头围小于未发生组(P<0.05)。发生组患者孕早期血清PP13、IGF-1水平低于未发生组(P<0.05)。经Pearson相关性检验分析,结果显示,GDM患者孕早期血清PP13、IGF-1水平分别与新生儿出生体重、头围呈正相关(r>0,P<0.05),且PP13与IGF-1呈正相关(r>0,P<0.05)。经logistic回归分析结果显示,PP13、IGF-1水平高是GDM患者发生FGR的保护因素(OR<1,P<0.05)。绘制ROC曲线结果显示,PP13、IGF-1预测GDM患者发生FGR的AUC>0.70,具有一定的预测价值。结论血清PP13、IGF-1水平与GDM患者发生FGR密切相关,可将PP13、IGF-1作为靶点制定治疗方案预防FGR的发生。Objective To investigate the relationship between serum placental protein 13(PP13),insulin-like growth factor-1(IGF-1)and fetal growth restriction(FGR)in patients with gestational diabetes mellitus(GDM).Methods A prospective case-control study was conducted on 135 patients with GDM admitted to Nanyang First People’s Hospital from February 2020 to September 2021.They were divided into occurrence group and non occurrence group according to whether FGR occurred.χ^(2) test and independent sample t test were used to compare the general data and laboratory indicators of the two groups.Pearson correlation test was used to analyze the correlation between the levels of serum PP13 and IGF-1 in early pregnancy and the birth weight and head circumference of newborns.Logistic regression was used to analyze the effect of serum PP13 and IGF-1 levels in early pregnancy on FGR in patients with GDM and the interaction between them.Results Among 135 patients with GDM,39 cases developed FGR,with an incidence of 28.89%.The birth weight and head circumference of newborns in the occurrence group were smaller than those in the non occurrence group(P<0.05).The levels of serum PP13 and IGF-1 in the early pregnancy of patients in the occurrence group were lower than those in the non occurrence group(P<0.05).The results of Pearson correlation test showed that the levels of serum PP13 and IGF-1 in early pregnancy of GDM patients were positively correlated with neonatal birth weight and head circumference respectively(r>0,P<0.05),and PP13 was positively correlated with IGF-1(r>0,P<0.05).Logistic regression analysis showed that high levels of PP13 and IGF-1 were protective factors for FGR in patients with GDM(OR<1,P<0.05).The results of ROC curve showed that PP13 and IGF-1 have a certain predictive value in predicting the AUC of FGR in patients with GDM>0.70.Conclusion The levels of serum PP13 and IGF-1 are closely related to FGR in patients with GDM.PP13 and IGF-1 can be used as targets to formulate treatment plans to prevent FGR.
