68例儿童慢性鼻-鼻窦炎血清SIgE结果分析

摘　　要：目的:分析68例儿童慢性鼻-鼻窦炎血清SIg E结果。方法:收集2015年7月-2018年7月赣南医学院第二附属医院的儿童慢性鼻-鼻窦炎(CRS)患者68例及健康儿童20例,统计分析两组人群血清SIgE阳性率、血清SIgE阳性变应原分布情况、血清SIgE阳性变应原种(类)数分布情况,分析儿童CRS人群血清SIgE阳性变应原前三种分级情况。结果:CRS组患儿的血清SIgE阳性率57.35%(39/68),高于健康儿童组的5.00%(1/20)(χ^(2)=17.085,P<0.05)。CRS组患儿血清SIgE阳性变应原分布前三位为户尘螨(24/39)、腰果(9/39)、虾(8/39)、蟹(8/39)、MX3(8/39)。CRS组患儿的血清SIgE阳性变应原种(类)数分布中,1种、2种、3种、≥4种比率分别为38.46%(15/39)、20.51%(8/39)、12.82%(5/39)、28.21%(11/39)。CRS组患儿的血清SIgE阳性户尘螨分级主要为2级,占50.00%;腰果分级主要为1级,占77.78%;蟹、虾分级主要为1、2级,均占50.00%;MX3分级主要为2级,占50.00%。CRS组患儿的血清SIgE阳性变应原前三种<3级、≥3级比率分别为80.70%、19.30%。CRS组患儿的血清SIgE阳性变应原户尘螨的分级<3级、≥3级比率分别为62.50%、37.50%。CRS组患儿的SIgE阳性户尘螨、腰果、蟹、虾、MX3<3级比率均高于≥3级。结论:儿童慢性鼻-鼻窦炎血清SIgE阳性率较健康儿童高,变应原分级主要<3级。Objective:To analyze the results of serum SIgE in 68 children with chronic rhinosinusitis.Method:The 68 children with CRS and 20 healthy children of the Second Affiliated Hospital of Gannan Medical College from July 2015 to July 2018 were collected,the positive rate of serum SIgE in the two groups,and distribution of serum SIgE positive allergens,and distribution of serum SIgE positive allergen species(class)were analyzed,and the first three grades of serum SIgE positive allergens in children with CRS population were analyzed.Result:The positive rate of serum SIgE in children with CRS was 57.35%(39/68),which was higher than 5.00%(1/20)in healthy children(χ^(2)=17.085,P<0.05).The distribution of top three serum SIgE positive allergens in children with CRS were house dust mites(24/39),cashew(9/39),shrimp(8/39),crab(8/39),and MX3(8/39).In the distribution of serum SIgE positive allergens(class)in children with CRS,1,2,3 and≥4 ratios were 38.46%(15/39),20.51%(8/39),12.82%(5/39)and 28.21%(11/39).The grade of SIgE positive house dust mites in CRS group was mainly grade 2,accounting for 50.00%.The main grading of cashew was grade 1,accounting for 77.78%.Crab and shrimp were classified as grade 1 and grade 2,all accounting for 50.00%.MX3 was mainly classified as grade 2,accounting for 50.00%.The ratios of the first three SIgE positive allergens grade<3 and≥3 in CRS group were 80.70%and 19.30%,respectively.In CRS group,the proportion of children with SIgE positive allergen dust mites of grade<3 and≥3 were 62.50%and 37.50%respectively.The rates of SIgE positive house dust mites,cashew nuts,crabs,shrimps and MX3 grade<3 in CRS group were higher than grade≥3.Conclusion:The positive rate of serum SIgE in children with chronic rhinosinusitis is higher than healthy children,and the allergen grade is mainly lower than grade 3.
