针灸、推拿结合药熨法治疗颈椎性肩周炎的临床效果

摘　　要：目的:观察和分析颈椎性肩周炎患者联合应用针灸、推拿及药熨法治疗的效果。方法:选取2020年5月-2022年4月永泰县总医院岭路分院收治的114例颈椎性肩周炎患者,根据患者就医顺序编号并以随机数字表法为分组工具,将所选患者随机分成对照组与观察组,均为57例。对照组采用针灸、推拿疗法,观察组采用针灸、推拿联合药熨法。比较两组临床治疗效果、视觉模拟评分法(VAS)及Constant-Murley肩关节功能评分表(CMS)评分、中医症候积分和炎症因子水平。结果:观察组总有效率高于对照组(P<0.05)。治疗后,观察组VAS评分低于对照组,CMS评分高于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组关节疼痛、关节沉重、关节酸胀及屈伸不利评分均低于对照组(P<0.05)。治疗后观察组白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平均低于对照组(P<0.05)。结论:颈椎性肩周炎患者联合应用针灸、推拿及药熨法治疗效果更加明显,可取得理想的抗炎及止痛效果,有助于促进患者肩关节功能改善,对于提升患者生活品质意义重大。Objective:To observe and analyze the effect of acupuncture,massage and medical ironing method in the treatment of cervical spondylotic periarthritis of shoulder.Method:A total of 114 patients with cervical spondylotic periarthritis of shoulder admitted to Linglu Branch of Yongtai County General Hospital from May 2020 to April 2022 were selected,numbered according to the order in which the patients were treated,used random number table method as grouping tool they were randomly divided into control group and observation group,with 57 patients in each group.The control group was treated with acupuncture and massage therapy,and the observation group was treated with acupuncture and massage combined with medical ironing method.The clinical effect,visual analog scale(VAS),Constant-Murley shoulder joint function scale(CMS)scores,traditional Chinese medicine syndrome scores and inflammatory factors were compared between the two groups.Result:The total effective rate of observation group was higher than that of control group(P<0.05).After treatment,VAS score of observation group was lower than that of control group,CMS score was higher than that of control group,the differences were statistically significant(P<0.05).After treatment,the scores of joint pain,joint heaviness,joint distension and adverse flexion and extension of observation group were lower than those of control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),hypersensitive C reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)of observation group were lower than those of control group(P<0.05).Conclusion:The combined application of acupuncture,massage and medical ironing method in patients with cervical spondylotic periarthritis of shoulder has more obvious effects,which can achieve ideal anti-inflammatory and analgesic effects,which is helpful to promote the improvement of shoulder joint function of patients,and is of great significance for improving the quality of life of patients.
