雌激素软膏局部联合盆底康复治疗围绝经、绝经后女性张力性尿失禁患者对盆底功能的影响

摘　　要：目的研究围绝经、绝经后女性张力性尿失禁患者采用雌激素软膏局部联合盆底康复治疗效果及对盆底功能的影响。方法选取51例围绝经、绝经后女性张力性尿失禁患者,根据随机数字表法将其分为对照组(25例)与观察组(26例)。对照组采用盆底康复治疗,观察组在对照组基础上采用雌激素软膏局部治疗。对比两组的尿失禁情况、盆底功能(盆底收缩压、盆底静息压、Ⅰ类肌纤维持续收缩压、Ⅱ类肌纤维持续收缩压),同时采用盆底障碍功能调查表(PFDI-20)和盆底障碍影响问卷(PFIQ-7)评估两组生活质量。结果两组治疗前72 h尿失禁次数和1 h尿失禁次数对比,差异无统计学意义(P>0.05);观察组治疗后的72 h尿失禁次数及1 h尿失禁次数均少于对照组(P<0.05)。观察组治疗后的盆底收缩压、盆底静息压、Ⅰ类肌纤维持续收缩压、Ⅱ类肌纤维持续收缩压水平均高于对照组(P<0.05)。观察组治疗后PFDI-20、PFIQ-7评分分别为(15.09±2.11)分、(26.28±4.06)分均低于对照组的(17.43±4.22)分、(29.76±6.39)分(P<0.05)。结论在围绝经、绝经后女性张力性尿失禁患者中采用雌激素软膏局部联合盆底康复治疗,能够有效减少尿失禁次数,改善盆底肌功能,提高生活质量。Objective To investigate the effect of estrogen ointment combined with pelvic floor rehabilitation on perimenopausal and postmenopausal female patients with tension urinary incontinence and the effect on pelvic floor function.Methods 51 perimenopausal and postmenopausal female patients with tension urinary incontinence were selected and divided into control group(25 cases)and observation group(26 cases)according to random number table method.The control group was treated with pelvic floor rehabilitation,and the observation group was treated with estrogen ointment on the basis of the control group.Urinary incontinence,pelvic floor function(pelvic floor systolic pressure,pelvic floor resting pressure,ClassⅠsustained systolic pressure,ClassⅡsustained systolic pressure)of the two groups were compared.Meanwhile,the pelvic floor dysfunction function questionnaire(PFDI-20)and the pelvic floor dysfunction impact questionnaire(PFIQ-7)were used to evaluate the quality of life of the two groups.Results There were no significant differences in the frequency of urinary incontinence 72 h and 1 h between the two groups before treatment(P>0.05).The frequency of urinary incontinence at 72 h and 1 h after treatment in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of pelvic floor systolic pressure,pelvic floor resting pressure,classⅠsustained systolic pressure and ClassⅡsustained systolic pressure in the observation group were higher than those in the control group(P<0.05).PFDI-20 and PFIQ-7 scores in the observation group were(15.09±2.11)points and(26.28±4.06)points,respectively,lower than(17.43±4.22)points and(29.76±6.39)points in the control group(P<0.05).Conclusion Local oestrogen ointment and pelvic floor rehabilitation treatment in postmenopausal women can effectively reduce the number of urinary incontinence,improve the pelvic floor muscle function,and improve the quality of l ife.
