基于“气络”“血络”理论探析胰岛素抵抗的病因病机及治疗

摘　　要：胰岛素抵抗是多种慢性病的发病基础。基于“气络”“血络”理论,提出从“络损”角度认识胰岛素抵抗,认为络脉运行周身,脉体纤细,易受侵害。络分气血,气络是脾气散精、肝气疏泄的通路,气络损则脾失散精,胰岛素不归正化及输布不力;肝失疏泄,或郁而化火耗伤津液,或亢而灼伤他脏。血络是传输精津液,进行物质交换与代谢的场所,血络损致液滞成瘀、痰积留浊,瘀痰浊缠绵胶着难治。气络损伤与血络损伤交互为害,导致胰岛素抵抗的发生发展。治疗上提出补气健脾、益气升提、畅气疏肝、活血化瘀的治则。Insulin resistance is the basis of many chronic diseases.Based on the theory of“Qi collateral”and“blood collateral”,it is proposed to understand insulin resistance from the perspective of“collateral damage”.Collateral are divided into Qi and blood,and Qi collateral are the pathways through which the temper Qi looses essence and liver Qi is dredged.If Qi collateral are damaged,the spleen loses essence,insulin is not normalized and infused,and the distribution of insulin is not effective;Or hyperactive and burn his dirty.The blood collateral are the place for the transmission of essence and body fluid,material exchange and metabolism.Damage to the blood collateral leads to fluid stagnation and stasis,phlegm accumulation and turbidity,which are difficult to cure.Qi collateral damage and blood collateral damage interact with each other,leading to the occurrence and development of insulin resistance.In terms of treatment,the principles of invigorating Qi and invigorating the spleen,invigorating Qi and lifting,smoothing Qi and soothing the liver,promoting blood circulation and removing blood stasis are put forward.
