温针灸联合脊柱微调手法治疗颈型颈椎病患者效果分析

摘　　要：目的:分析温针灸联合脊柱微调手法治疗颈型颈椎病患者的效果。方法:选取2021年3月—2022年3月天津市北辰医院收治的88例颈型颈椎病患者通过红蓝双色球法随机分为传统组(44例)、联合组(44例)。传统组应用脊柱微调手法治疗,联合组则加用温针灸治疗。比较2组临床疗效和不良事件发生情况,治疗前后的颈椎功能、疼痛程度、神经功能及日常生活能力,治疗前及治疗3个月后的颈椎曲度及颈肌表面肌电信号(sEMG)。结果:相比传统组的79.55%,联合组的总有效率为97.73%,显著更高(P<0.05)。治疗后2组运动功能、主观感觉、生活能力及日本骨科学会量表(JOA)评分均显著提高,相比传统组,联合组显著更高(P<0.05)。治疗后2组颈痛量表(NPQ)、神经功能缺损程度评分量表(CSS)、颈肩痛相关功能障碍量表(NDI)评分均显著降低,相比传统组,联合组显著更低(P<0.05)。治疗后2组颈椎曲度、斜方肌sEMG、胸锁乳突肌sEMG均显著提高,相比传统组,联合组显著更高(P<0.05)。2组不良事件发生率相近(P>0.05)。结论:温针灸联合脊柱微调手法治疗颈型颈椎病患者效果显著,明显改善患者颈椎功能、疼痛程度、神经功能及日常生活能力,提高患者颈椎曲度sEMG,且安全性较高。Objective:To analyze the effect of warm needling moxibustion combined with fine-tuning manipulation of the spine in the treatment of patients with cervical spondylosis.Methods:88 patients with cervical spondylosis who met the criteria were selected and randomly divided into the traditional group(44 cases)and the combined group(44 cases)by the red and blue double-color ball method.The traditional group was treated with spinal fine-tuning manipulation,while the combined group was treated with warm acupuncture.The clinical efficacy and adverse events,cervical spine function,pain degree,nerve function and daily living ability before and after treatment,cervical curvature and surface electromyographic signal(sEMG)of cervical muscle(sEMG)before and after treatment were compared between the two groups.Results:Compared with 79.55%of the traditional group,the total effective rate of the combination group was 97.73%,which was significantly higher(P<0.05).After treatment,the motor function,subjective sensation,life ability and Japanese Orthopedic Association(JOA)scores in both groups were significantly improved,and compared with the traditional group,the combination group was significantly higher(P<0.05).After treatment,the scores of Neck Pain Scale(NPQ),Neurological Impairment Scale(CSS),and Neck and Shoulder Pain-Related Disability Inventory(NDI)in the two groups were all significantly reduced,and compared with the traditional group,the joint group was significantly lower(P<0.05).After treatment,the cervical curvature,trapezius muscle sEMG,and sternocleidomastoid muscle sEMG in both groups were significantly improved,and compared with the traditional group,the combination group was significantly higher(P<0.05).The incidence of adverse events in the two groups was similar(P>0.05).Conclusion:Warming needling moxibustion combined with spinal fine-tuning manipulation has a significant effect on patients with cervical spondylosis.It can significantly improve the cervical spine function,pain level,nerve function and daily life
