摘  要:目的讨论CT灌注成像对颞浅动脉-大脑中动脉搭桥术后脑血流的评估及与改良Rankin量表(mRS)、卡氏功能状态评分(KPS)、改良Barthel指数(MBI)评分的相关性。方法选择2020年1月至2022年5月期间我院收治的60例烟雾病患者,所有患者均采取颞浅动脉-大脑中动脉搭桥术治疗,并在手术前和手术后接受CT灌注成像,评估临床疗效。结果术后3个月,患者mRS、KPS、MBI评分均比术前、术后7 d高(P<0.05),而mRs低于术前及术后7 d;术后3个月,患者相对脑血流量(rCBF)、相对脑血流量容量(rCBV)比术前、术后7 d高(P<0.05),相对平均通过时间(rMTT)和相对达峰时间(rTTP)指标比术前、术后7 d低(P<0.05)。结论颞浅动脉-大脑中动脉搭桥可有效改善烟雾病患者临床症状,提高其生活质量,促进缺血区域侧支循环代偿,改善颅内缺血,而CT灌注成像能有效评估患者血流动力学,对患者病情恢复具有重要价值。Objective To discuss the evaluation of CT perfusion imaging on cerebral blood flow after superficial temporal artery-middle cerebral artery bypass grafting and its correlation with mRS,KPS and MBI scores.Methods A total of 60 patients with moyamoya disease admitted to our hospital from January 2020 to May 2022 were selected.All patients were treated with superficial temporal artery-middle cerebral artery bypass surgery and underwent CT perfusion imaging before and after surgery.The clinical efficacy was assessed.Results At 3 months after the operation,the KPS,and MBI scores of patients were higher than those before the operation and at 7 days after the operation,while mRS scores were lower(P<0.05).The rCBF,rCBV indexes were higher(P<0.05),while the rMTT and rTTP indexes were lower than those before the operation and 7 days after the operation(P<0.05).Conclusion Superficial temporal artery-middle cerebral artery bypass can effectively improve the clinical symptoms of patients with moyamoya disease,improve their quality of life,promote the compensation of collateral circulation in the ischemic area,and improve intracranial ischemia.CT perfusion imaging can effectively evaluate the blood flow of patients Dynamics is of great value to the recovery of the patient′s condition.
