摘  要:目的比较浮针与毫针对实验性腰椎间盘突出症(LIDH)兔步态功能、β-内啡肽(β-EP)及炎症因子表达的影响。方法选择新西兰家兔40只,随机分为空白组、模型组、浮针组和毫针组,每组各10只。模型组、浮针组和毫针组采用自制的LIDH造模器制备家兔LIDH模型。造模后1 d,浮针组给予浮针治疗,隔日1次,共治疗5次;毫针组行毫针治疗,每日1次,共治疗10次。干预结束后1 d,比较造模前、造模后及治疗后家兔步态功能;提取血清及髓核,采用酶联免疫吸附法检测血清β-EP、前列腺素E_(2)(PGE_(2)),髓核肿瘤坏死因子α(TNF-α)、白介素6(IL-6)水平。结果除空白组外,其他三组造模后家兔步态评分低于造模前(P<0.01)。治疗后,浮针组、毫针组家兔步态评分高于造模后(P<0.01)。浮针组治疗前后步态评分差值显著高于空白组、模型组和毫针组(P<0.01)。浮针组、毫针组血清β-EP水平高于模型组,血清PGE_(2)、髓核TNF-α、IL-6水平低于模型组(P<0.05),且浮针组的变化幅度均大于毫针组。结论浮针、毫针治疗LIDH家兔均有效,浮针疗效优于毫针,且浮针治疗的频率少于毫针,其机制可能是浮针配合导引功能活动,进一步诱导β-EP的释放,从而加速减轻炎症反应,减轻疼痛。Objective To compare the effects of Fu’s subcutaneous needling(FSN)acupuncture and filiform needle acupuncture on gait function,and expressions ofβ-endorphin(β-EP)and inflammatory factors in rabbits with experimental lumbar intervertebral disc herniation(LIDH).Methods A total of 40 New Zealand rabbits were randomly divided into the blank,model,FSN and filiform needle groups,with 10 rabbits in each group.The rabbit LIDH model was established using the self-made LIDH model maker in the model group,the FSN group and the filiform needle group.One day after modeling,the rabbits in the FSN group were treated with FSN acupuncture once every other day,for a total of five times.The rabbits in the filiform needle group were treated with filiform needle acupuncture once a day,for a total of 10 times.One day after the intervention,the gait functions of rabbits before and after modeling as well as after treatment were compared.Serum and nucleus pulposus were extracted,and enzyme-linked immunosorbent assay(ELISA)was used to detect serumβ-EP,prostaglandin E_(2)(PGE_(2)),nucleus pulposus tumor necrosis factorα(TNF-α)and interleukin-6(IL-6).Results Except for the blank group,the gait scores of rabbits in the other three groups after modeling were lower than those before modeling(P<0.01).After treatment,the gait scores of rabbits in the FSN group and the filiform needle group were higher than those after modeling(P<0.01).The difference in gait scores before and after treatment in the FSN group was significantly higher than that in the blank group,model group,and filiform needle group(P<0.01).Serum of FSN group and filiform needle groupβ-EP level is higher than that of model group,serum PGE_(2),nucleus pulposus TNF-α、The level of IL-6 in the FSN group was lower than that in the model group(P<0.05),and the amplitude of change in the FSN group was greater than that in the filiform needle group.Conclusion Both FSN acupuncture and filiform needle acupuncture are effective in the treatment of rabbits with LIDH.The efficacy of
