摘　　要：目的探讨创伤性四肢骨干骨折患者外固定架联合内固定术治疗的临床效果及对患者预后、QOL评分的影响。方法选取2018年6月至2021年6月我院创伤骨科中心收治的40例创伤性四肢骨干骨折患者,随机分为对照组和研究组各20例,对照组应用内固定术治疗,研究组应用外固定架联合内固定术治疗。观察分析两组恢复优良率、并发症、手术结果、两项评分、炎性因子及骨代谢指标。结果与对照组相比,研究组恢复优良率更高,并发症发生率更低(P<0.05);与对照组相比,研究组骨折愈合时间、下床活动时间及住院时间更短(P<0.05);与对照组相比,研究组QOL评分更高,VAS评分更低(P<0.05);与对照组相比,研究组CRP、TNF-α及IL-1水平更低(P<0.05);与对照组相比,研究组BGP、BALP及PICP水平更高(P<0.05)。结论创伤性四肢骨干骨折患者应用外固定架与内固定术联合治疗效果更为显著,可有效改善其预后,值得应用。Objective To investigate the clinical effect of external fixation combined with internal fixation in patients with traumatic limb fractures and shaft fracture and its influence on the prognosis and QOL score of patients.Methods A total of 40 patients with traumatic limb fractures and shaft fracture admitted to the trauma orthopedic center of our hospital from June 2018 to June 2021 were randomly divided into the control group and the study group with 20 cases each.The control group was treated with internal fixation,and the study group was treated with external fixation and internal fixation treatment.Observe and analyze the recovery rate,complications,surgical results,two scores,inflammatory factors and bone metabolism indicators in the two groups.Results Compared with the control group,the study group had higher recovery rate and lower complication rate(P<0.05);compared with the control group,the fracture healing time,the time to get out of bed and the hospitalization time of the study group were shorter(P<0.05).Compared with the control group,the QOL score of the study group was higher,and the VAS score was lower(P<0.05);compared with the control group,the study group had lower levels of CRP,TNF-αand IL-1(P<0.05).Compared with the control group,the study group had higher levels of BGP,BALP and PICP(P<0.05).Conclusion The combined treatment effect of external fixation and internal fixation for patients with traumatic limb fractures and shaft fracture is more significant,which can effectively improve their prognosis and is worthy of application.
