补肾固齿丸对中老年慢性牙周炎临床疗效及血清免疫球蛋白、炎症因子水平影响研究

摘　　要：目的探讨补肾固齿丸对中老年慢性牙周炎患者疗效及对血清免疫球蛋白、炎症因子水平的影响性。方法按治疗前后顺序选择2018年12月—2020年12月共116例中老年慢性牙周炎患者为研究对象,单号就诊为对照组,牙周基础治疗后口服阿西莫林胶囊+甲硝唑片1周,双号就诊为研究组,在对照组基础上口服补肾固齿丸3个月,比较两组治疗前、治疗3个月后在免疫球蛋白、龈沟液炎症因子、探诊深度、菌斑指数、附着丧失水平变化情况。总结疗效。结果①两组治疗3个月后探诊深度、菌斑指数、附着丧失水平较治疗前均显著下降,差异有统计学意义(P<0.05),且治疗3个月后研究组以上指标显著低于对照组(P<0.05)。②两组治疗3个月后IgG水平较治疗前均显著上升,差异有统计学意义(P<0.05),且治疗3个月后研究组免疫球蛋白G(immunoglobulin G,IgG)显著高于对照组(P<0.05),但免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白M(immunoglobulin M,IgM)水平治疗前、治疗3个月后比较差异无统计学意义(P>0.05)。③两组治疗3个月后龈沟液白细胞介素-6(interleukin-6,IL-6)、白细胞介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、C反应蛋白(creactiveprotein,CRP)较治疗前均显著下降,白细胞介素10(interleukin 10,IL-10)较治疗前显著上升,差异有统计学意义(P<0.05),且治疗3个月后研究组以上指标显著优于对照组(P<0.05)。④治疗3个月后对照组显效率41.38%(24/58)、总有效率77.59%(45/58),研究组显效率55.17%(32/58)、总有效率89.65%(52/58),研究组显著高于对照组,比较差异有统计学意义(P<0.05)。结论补肾固齿丸可通过抑制中老年慢性牙周炎龈沟液炎症水平,提高机体免疫应答,从而提高疗效,改善症状。Objective To explore the therapeutic effect of Bushen Guchi Pill(补肾固齿丸)on the elderly patients with chronic periodontitis and its influence on the levels of serum immunoglobulin and inflammatory factors.Methods A total of 116 cases of chronic periodontitis of middle-aged and elderly patients from December 2018 to December 2020 were selected as the study objects according to the sequence before and after treatment.The patients with the odd numbers visit were as the control group.After the basic periodontal treatment,the patients were treated with acimolin capsule+metronidazole tablet for one week.Those with the even number visit were as the study group.On the basis of the control group,the patients were treated with Bushen Guchi Pill for three months.Before and after the treatment,the patients in the two groups were compared in terms of immunoglobulin,inflammatory factors in gingival crevicular fluid,depth of exploration,plaque index and loss of attachment after 3 months of treatment.The curative effect was summarized.Results①After 3 months of treatment,the depth of detection,plaque index and the loss of attachment in the two groups were significantly lower than those before treatment(P<0.05),and the above indicators in the study group were significantly lower than those in the control group(P<0.05).②The level of IgG in the study group was significantly higher than that in the control group(P<0.05),but there was no significant difference in the levels of IgA or IgM before and after treatment(P>0.05).③After 3 months of treatment,the levels of IL-6,IL-1β,TNF-αand C-reactive protein(CRP)in gingival crevicular fluid of the two groups were significantly lower than those before treatment,and IL-10 was significantly higher than that before treatment.The difference was statistically significant(P<0.05),and the above indexes of the study group were significantly better than those of the control group after 3 months of treatment(P<0.05).④After 3 months of treatment,the effective rate of the control group
