益气养阴活血汤辅助激光治疗糖尿病视网膜病变对sIGAM-1、CTGF水平的影响

摘　　要：目的研究益气养阴活血汤辅助激光治疗糖尿病视网膜病变(diabetic retinopathy,DR)对可溶性细胞黏附分子(soluble intercellular adhesion molecule-1,sICAM-1)、结缔组织生长因子(connective tissue growth factor,CTGF)水平的影响。方法选择河南省中医院收治的60例DR患者作为研究对象,将其随机分为观察组和对照组,各30例,其中对照组使用视网膜激光光凝进行治疗,观察组在对照组基础上加服益气养阴活血汤。观察两组临床疗效,比较两组治疗前1 d和治疗后3个月中医证候积分、眼部相关指标(黄斑厚度、血管瘤体积、视野灰度值、出血斑面积)、血清学指标[sICAM-1、CTGF、肿瘤坏死因子(tumour necrosis factor-α,TNF-α)、转化生长因子-β(transforming growth factor-β,TGF-β)、核因子-κB(nuclear factor-κB,NF-κB)],观察并记录治疗期间不良反应发生情况。结果与对照组(73.33%)相比,观察组(93.33%)治疗总有效率较高(P<0.05);治疗后,观察组视物昏花、眼睛干涩、五心烦热、头晕头痛、口渴喜饮证候积分及总分均有所降低,对照组视物昏花、眼睛干涩证候积分及总分有所降低(P<0.05);治疗后,与对照组相比,观察组视物昏花、眼睛干涩、五心烦热、头晕头痛、口渴喜饮证候积分及总分较低(P<0.05);治疗后,两组黄斑厚度、血管瘤体积、视野灰度值、出血斑面积均有所下降(P<0.05);治疗后,与对照组相比,观察组黄斑厚度、血管瘤体积、视野灰度值、出血斑面积较小(P<0.05);治疗后,两组sICAM-1、CTGF、TNF-α水平均有所降低(P<0.05);治疗后,与对照组相比,观察组sICAM-1、CTGF、TNF-α水平较低(P<0.05);治疗后,两组TGF-β、NF-κB水平均有所降低(P<0.05);治疗后,与对照组相比,观察组TGF-β、NF-κB水平较低(P<0.05);治疗期间,观察组发生食欲减退1例,暂时性高眼压2例,对照组暂时性高眼压1例,两组不良反应发生率差异无统计学意义(χ2=0.268,P=0.605)。结论Objective To study the effects of Yiqi Yangyin Huoxue Decoction(益气养阴活血汤)on the levels of soluble cell adhesion molecule(sICAM-1)and connective tissue growth factor(CTGF)in diabetic retinopathy(DR).Methods Sixty DR patients treated in Henan Hospital of Traditional Chinese Medicine were selected as the research objects,and they were randomly divided into the observation group and the control group,30 cases in each group.The control group was treated with retinal laser photocoagulation,and the observation group added Yiqi Yangyin Huoxue Decoction on the basis of control group.The clinical efficacy of the two groups was observed.TCM syndrome scores,eye related indexes(macular thickness,hemangioma volume,gray value of visual field,hematologic area),and serological indexes[sICAM-1,CTGF,tumor necrosis factor(TNF-α),transforming growth factor-β(TGF-β),nuclear factor-κB(NF-κB)]were compared between the two groups 1 d before treatment and 3 months after treatment.The incidence of adverse reactions during treatment was observed and recorded.Results Compared with that of the control group(73.33%),the total effective rate of the observation group(93.33%)was higher(P<0.05).After treatment,the score and total score of visual blackout,dry eyes,five-upset fever,dizziness and headache,thirst and drinking syndrome in the observation group were decreased,while the score and total score of visual blackout,dry eyes in the control group were decreased(P<0.05).After treatment,compared with those of the control group,the scores and total scores of the syndrome of fainting vision,dry eyes,five irritability,dizziness,headache,thirst and drinking preference in the observation group were lower(P<0.05).After treatment,macular thickness,hemangioma volume,visual field gray value and bleeding macular area were decreased in both groups(P<0.05).After treatment,compared with those of the control group,the macular thickness,hemangioma volume,visual field gray value and bleeding spot area in the observation group were smaller(P<0.0
