失能老年人护理模式需求影响因素的多层线性模型研究

摘　　要：[目的]探讨影响失能老年人护理模式需求的因素,为合理配置护理资源提供依据,为我国长期护理体系的建设与完善提供有价值的信息。[方法]基于2018年中国老年健康影响因素跟踪调查,采用多层线性模型了解失能老年人护理模式需求的影响因素。[结果]多层线性模型结果显示,748名失能老年人中,护理补贴越多、子女居住距离越远、慢性病种类越多的老年人越倾向于选择社会化护理模式(P<0.05);护理机构床位数越多、失能等级越严重、受教育程度越高的老年人越倾向于选择居家护理模式(P<0.05);失能等级、慢性病种类和护理补贴之间交互效应显著为正(P<0.05);子女居住地距离和护理补贴之间交互效应显著为负(P<0.05)。[结论]护理机构床位数、失能等级、受教育程度对失能老年人护理模式需求有负向影响,护理补贴、子女居住距离、慢性病种类对失能老年人护理模式需求有正向影响,且护理补贴与子女居住距离、失能等级、慢性病种类之间存在交互效应。Objective This paper explores the factors influencing the demand of nursing mode of elderly people with disabilities,provides basis for rationally allocating nursing resources and provides valuable information for constructing and improving long-term nursing system in China.Methods Based on the Chinese longitudinal healthy longevity survey(CLHLS)in China in 2018,the hierarchical linear model was used to understand the factors affecting the needs of nursing mode of elderly people with disabilities.Results The results of hierarchical linear model showed that among 748 older adults with disabilities,the elderly who had more nursing subsidies and more types of chronic diseases and whose children lived further were more inclined to choose social nursing mode(P<0.05).More beds in nursing institutions,the elderly with higher disability level and higher education level tended to choose home-based care mode(P<0.05).The interaction effect among disability level,types of chronic diseases and nursing subsidies was significantly positive(P<0.05).The interaction effect between distance of children's residence and nursing subsidies was significantly negative(P<0.05).Conclusions Beds in nursing institutions,disability level and education level have a negative impact on the demand of nursing mode of elderly people with disabilities,while nursing subsidies,distance of children's residence and types of chronic diseases have a positive impact on the demand of nursing mode of elderly people with disabilities.Moreover,there are interaction effects between nursing subsidies and distance of children's residence,disability level,and types of chronic diseases.
