基于结构变动度的新疆某省级三甲医院住院费用分析

摘　　要：[目的]分析新疆某省级三甲医院住院费用结构变动情况,探讨主要的影响因素,为优化住院费用结构、提高医务人员积极性提供参考。[方法]收集新疆某省级三甲医院2016-2021年各项住院费用,采用结构变动度分析方法,分析和评价住院费用的变动度、变动方向、贡献率和拉动力。[结果]2016-2021年,该三甲医院住院费用总结构变动度为36.02%,年均变动度为7.20%;住院费用中,药品类费用结构呈负向变动趋势,而技术性的医疗服务费用呈正向变动趋势。住院费用的拉动力主要来自治疗费(26.51%)、手术费(17.58%)和材料费(10.89%)。[结论]药品零加成政策实施后,该三甲医院的住院费用结构逐渐合理化。政府应继续推进医疗服务价格改革,适当提高医疗服务过程中技术性的占比,合理降低药品及材料费;医院应该持续优化结构,加大新技术的力度,建立合理的控费机制,提高医院的运转效率。Objective To analyze the structure variation of hospitalization expenses in a provincial tertiary hospital in Xinjiang,and to explore the main influencing factors,so as to provide reference for optimizing the structure of hospitalization expenses and improving the enthusiasm of medical staff.Methods Hospitalization expenses of a provincial tertiary hospital in Xinjiang from 2016 to 2021 were collected to analyze and evaluate the degree and direction of variation,contribution rate and driving force of hospitalization expenses by using the degree of structure variation.Results From 2016 to 2021,the total degree of structure variation of hospitalization expenses in the tertiary hospital was 36.02%and the average annual variation was 7.20%.The structure of drug expenses showed a negative trend,while the cost of technical medical services showed a positive trend among the hospitalization expenses.The main driving forces of hospitalization expenses were treatment expenses(26.51%),operation expenses(17.58%)and materials expenses(10.89%).Conclusions The structure of the hospitalization expenses in this tertiary hospital is gradually rationalized after the policies of zero mark-ups on drugs.The government should reform the price of medical services continuously,appropriately increase the proportion of technology in medical services,and reasonably reduce the cost of drugs and materials.Hospitals should optimize the structure continuously,increase new technology,establish a mechanism of reasonable cost control and improve the efficiency of hospital.
