摘　　要：目的探讨基于湿性愈合理念指导下海桐皮汤湿敷对下肢静脉性溃疡愈合的疗效观察。方法从2017年6月—2019年7月入该院接受治疗下肢静脉性溃疡患者中选取80例,应用信封法进行分组,将患者分为对照组与研究组。对照组患者采用常规抗感染治疗,研究组患者使用海桐皮汤湿敷治疗。对比两组患者临床症状体征积分、临床疗效、两组炎性指标状况及血管内皮生长因子(VEGF)水平。结果治疗干预前两组患者肉芽上皮新生、出血、水肿、创面渗液、疼痛程度症状总评分差异不显著,无统计学意义(P>0.05),经治疗,研究组患者临床症状体征积分均低于对照组(P<0.05);经治疗干预,研究组白细胞计数(WBC)、超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)水平比对照组更低,VEGF水平比对照组更高,差异具有统计学意义(P<0.05)。结论基于湿性愈合理念指导下海桐皮汤湿敷治疗下肢静脉性溃疡,可显著提升临床疗效,降低临床症状体征积分,同时可调节机体炎性因子水平,提升VEGF水平,促进创面愈合,应用效果较为满意,可考虑普及。Objective To explore the curative effect of Haitongpi Decoction(海桐皮汤)wet compress on the healing of venous leg ulcers based on the concept of wet healing.Methods From June 2017 to July 2019,80 patients with venous ulcers of lower extremities who were admitted to the hospital for treatment were selected and divided into control group and research group by envelope method.The control group received conventional anti-infective treatment,while the research group was treated with Haitongpi Decoction wet compress.The clinical symptoms and signs scores,clinical efficacy,inflammatory index status and vascular endothelial growth factor(VEGF)level of the two groups were compared.Results The total scores of granulation epithelial neoplasia,hemorrhage,edema,wound exudate and pain degree were not significantly different between the two groups before treatment intervention,with no statistical significance(P>0.05).In the control group after treatment intervention,the levels of white blood cells(WBC),high sensitivity C-reactive protein(hs-CRP)and interleukin-6(IL-6)in the study group were lower than those of the control group,and the VEGF levels were higher than those of the control group(P<0.05).ConclusionThe guidance of Haitongpi Decoction wet compress based on the concept of wet healing can significantly improve the clinical efficacy,reduce the clinical symptoms and signs,and at the same time can adjust the levels of inflammatory factors,increase the level of VEGF and promote the wound healing.The application effect is relatively satisfactory,popularization can be considered.
