补阳还五汤联合脂必泰治疗2型糖尿病远端对称性多发性神经病变气虚血瘀证的临床观察

摘　　要：目的 观察补阳还五汤联合脂必泰治疗2型糖尿病远端对称性多发性神经病变(DSPN)气虚血瘀证的临床疗效。方法 80例2型糖尿病并发远端对称性多发性神经病变(DSPN)气虚血瘀证患者随机分成观察组及对照组,各40例,对照组采用积极控制血糖、营养神经、改善微循环等基础治疗,口服脂必泰0.24 g,每日2次;观察组在相同于对照组的基础上,加用口服补阳还五汤治疗。两组疗程4周,观察两组治疗前后血脂、同型半胱氨酸(HCY)、腓总神经(MNCV)、正中神经(SNCV)、TCSS评分、中医证候评分,以及中医临床疗效比率。结果 观察组总有效率87.5%(35/40),对照组总有效率65%(26/40),观察组优于对照组,差异有统计学意义(P<0.05);两组血脂、同型半胱氨酸(HCY)、腓总神经MNCV、正中神经SNCV、TCSS评分、中医证候评分等指标在同组间治疗后与治疗前比较,差异有统计学意义(P<0.05);两组在治疗后组间各指标比较,差异有统计学意义(P<0.05)。结论 补阳还五汤联合脂必泰治疗2型糖尿病远端对称性多发性神经病变(DSPN)气虚血瘀证疗效确切。Objective To observe the clinical efficacy of Buyang Huanwu Decoction(补阳还五汤)combined with Zhibitai(脂必泰)in the treatment of type 2 diabetes with distal symmetric polyneuropathy(DSPN)of Qi deficiency and blood stasis syndrome.Methods Eighty patients with type 2 diabetes complicated with distal symmetric polyneuropathy(DSPN)of Qi deficiency and blood stasis syndrome were randomly divided into the observation group and the control group,40 cases in each group.The control group adopted the basic treatment such as actively controlling blood sugar,nourishment nerve,improving microcirculation and so on,taking Zhibitai O.24 gorally,twice a day.The observation group was treated with Buyang Huanwu Decoction on the basis ofthe control group.The two groups were treated for 4 weeks.The blood lipid,homocysteine(Hcy),motor nerve conduction velocity(MNCV)of the general peroneal nerve,sensory nerve conduction velocity(SNCV)of median nerve,Toronto clinical scoring system(TCSS)score,syndrome score of TCM and the ratio of clinical efficacy of TCM were observed before and after treatment.Results The total effective rate of the observation group was 87.5%(35/40)and that of the control group was 65%(26/40)and the difference was significant(P<0.05).There were significant differences in blood lipid,Hcy,MNCV of the general peroneal nerve,SNCV of median nerve,TCSS score and TCM syndrome score between the two groups before and after treatment(P<O.05).There was significant difference between the two groups after treatment(P<0.05).Conclusion Buyang Huanwu Decoction combined with Zhibitai is effective in treating type 2 diabetes withDSPN of Qi deficiency and blood stasis syndrome.
