“标本配穴”电针对肥胖2型糖尿病患者糖脂代谢和胰岛素功能的改善作用

摘　　要：目的 观察不同部位腧穴配伍对肥胖2型糖尿病患者糖脂代谢及胰岛素敏感性相关指标的改善作用。方法 将212例肥胖2型糖尿病患者随机分为单纯针刺组(A组,53例,脱落1例)、腹部电针组(B组,53例)、四肢电针组(C组,53例)和标本配穴电针组(D组,53例,脱落1例)共4组。A组选取:关元、中脘、双侧天枢、足三里、丰隆穴给予单纯针刺治疗;B组选取关元、中脘、双侧天枢穴给予电针治疗;C组选取双侧足三里、丰隆穴给予电针治疗;D组取穴同单纯针刺组并给予电针治疗,共治疗4周。观察治疗前后4组患者肥胖指标、糖脂代谢指标和胰岛功能的变化。结果 与治疗前比较,治疗后4组体质量(W)、体质量指数(BMI)、体脂肪率(FAT)、空腹血糖(FBG)、餐后2h血糖(2 h PBG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)和胰岛素抵抗指数(HOMA-IR)均显著降低,高密度脂蛋白胆固醇(HDL-C)、空腹胰岛素(FINS)和胰岛素分泌指数(HOMA-β)显著升高(P<0.05),且D组的改善水平明显优于其余3组(P<0.05)。结论 “标本配穴”电针法治疗肥胖2型糖尿病的临床疗效确切,可明显改善肥胖、降低糖脂代谢和胰岛素敏感性水平,值得推广运用。Objective To observe the effect of acupoint compatibility in different parts on glucose and lipid metabolism and insulin sensitivity in obese patients with type 2 diabetes mellitus.Methods A total of 212 obese patients with type 2 diabetes mellitus were randomly divided into four groups: sham electroacupuncture group(group A,53 cases, 1 case of falling off),abdominal electroacupuncture group(group B,53 cases),limbs electroacupuncture group(group C,53 cases)and specimen matching electroacupuncture group(group D,53 cases, 1 case of falling off).Group A:Guanyuan(RN4),Zhongwan(RN12),bilateral Tianshu(ST25),Zusanli(ST36)and Fenglong(ST40)were treated with acupuncture alone.Group B was treated with electroacupuncture at Guanyuan(RN4),Zhongwan(RN12)and bilateral Tianshu(ST25).Group C was treated with electroacupuncture at Zusanli(ST36)and Fenglong(ST40).Group D was treated with electroacupuncture for 4 weeks.The changes of obesity index, glucose and lipid metabolism index and islet function were observed before and after treatment.The changes of obesity index, glucose and lipid metabolism index and islet function were observed before and after treatment.Results Compared with those before treatment, the body mass(W),body mass index(BMI),body fat rate(fat),fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),fasting insulin(FINS)and insulin secretion index(HOMA-β),high density lipoprotein cholesterol(HDL-C)and insulin resistance index(HOMA-IR)of the four groups after treatment were significantly increased and those of group D were significantly higher than those in the other three groups(P<0.05).Conclusion The clinical effect of electroacupuncture on obesity type 2 diabetes is definite.It can significantly improve obesity, reduce glucose and lipid metabolism and insulin sensitivity, and is worthy of promotion and application.
