从健脾祛湿法调节肠道菌群探讨艾滋病相关性腹泻临床机制

摘　　要：脾脏——“后天之本”、“气血生化之源”,在机体中占有极为重要的位置。HIV病毒侵袭人体后,脾胃首先受损,若脾运失常、脾气不和,湿浊由生,艾滋病患者则会出现腹泻的临床症状。有关本病论治,临床上多采取健脾益气、祛湿止泻的治法。研究显示,肠道菌群失调情况在艾滋病患者中普遍存在[1],腹泻的发生和肠道菌群失调有着密切的关系。故笔者以肠道菌群和中医学“脾”的相关理论知识作为研究背景,运用脾—胃—肠之间的相关理论,结合健脾祛湿法治疗以脾气虚为主的脾虚湿盛型HRD的临床实践以及机理内涵,探讨艾滋病相关性腹泻的治疗思路,以平衡肠道菌群为主要目的,改善艾滋病相关性腹泻的临床症状,达到维持艾滋病患者体内肠道菌群的平衡,以防治HRD的发生,为今后的临床防治提供新的方向理论依据。The spleen,as the“acquired origin”and“the source of Qi and blood”,occupies a very important position in the body.After the human immunodeficiency virus(HIV)attacks the human body,the spleen and stomach are first damaged.If the spleen is abnormal,it will cause dampness and turbidity.HIV/acquired immune deficiency syndrome(AIDS)will appear the clinical symptoms of diarrhea.In clinic,it is often treated by the method of invigorating spleen and Qi,eliminating dampness and stopping diarrhea.Studies have shown that intestinal flora disorders are commonin AIDS patients,and the occurrence of diarrhea is closely related to intestinal flora disorders.Therefore,taking the intestinal flora and TCM“spleen”theory as the research background,using the relevant theory of spleen-stomach-intestines and combining theclinical practice and mechanism of clearing dampness treatments for spleendeficiency and dampness excess type of HIV/AIDS related diarrhea(HRD),the author explored the treatment of HRD,with the main purpose of balancing the intestinal florato improve the clinical symptoms of HRD,maintain the balance of intestinal flora in AIDS patients,prevent HRD and provide a new theoretical basis for clinical prevention and treatment in the future.
