吴茱萸汤结合倍他司汀治疗胃虚肝寒性眩晕临床观察

摘　　要：目的 评价吴茱萸汤结合倍他司汀治疗胃虚肝寒性眩晕的临床观察。方法 研究对象65例选自2017年6月—2022年2月北京中医医院顺义医院收治的眩晕症患者,依据治疗方法的不同,将患者分为观察组(33例)和参照组(32例),两组患者入院后均给与常规治疗,给与参照组患者盐酸倍他司汀注射液治疗,将10 mg倍他司汀和250 mL的0.9%氯化钠注射液相融合,静脉滴注,1次/d,连续治疗2周。观察组采用盐酸倍他司汀注射液结合吴茱萸汤治疗,随证加减,连续治疗2周。比较两组治疗前后胃虚肝寒性眩晕症的临床中医证候积分,比较两组治疗前后眩晕障碍评估量表(DHI)评分、眩晕评定量表的评分系统(DARS)评分变化;比较治疗疗效。结果 观察组治疗总有效率87.88%(29/33),参照组治疗总有效率65.63%(21/32),参照组治疗总有效率低于观察组(P<0.05);两组治疗后眩晕、头痛、恶心呕吐和平衡障碍评分均较治疗前降低,观察组治疗后眩晕、头痛、恶心呕吐和平衡障碍评分低于参照组(P<0.05);观察治疗后DARS评分和DHI评分降低幅度优于参照组(P<0.05)。结论 吴茱萸汤结合倍他司汀可改善眩晕症的临床症状,降低中医证候积分,改善脑部血流,治疗疗效和生存质量得以大幅度提高,值得临床进一步研究探讨。Objective To evaluate the clinical observation of Wuzhuyu Decoction(吴茱英汤)combined with betastine in the treatment of stomach deficiency and liver cold type of vertigo.Methods Sixty-five patients with vertigo admitted to the hospital from June 2017 to February 2022 were divided into observation group and reference group according to different treatment methods,with 33 cases in observation group and 32 cases in reference group.The patients in both groups were given routine treatment after admission,and the patients in the reference group were given betahistine hydrochloride injection.10 mg betahistine and 250 mL 0.9%sodium chloride injection were combined,intravenous drip,once a day,for two consecutive weeks.The observation group was treated with betahistine hydrochloride injection combined with Wuzhuyu Decoction.The patients were treated continuously for 2 weeks.The clinical TCM syndrome score before and after treatment,the changes of vertigo disorder assessment scale(DHI)score and vertigo assessment scale scoring system(DARS)score between the two groups before and after treatment and the therapeutic effects were compared.Results The total effective rate of the observation group was 87.88%(29/33),while that of the reference group was 65.63%(21/32).The total effective rate of the reference group was lower than that of the observation group(P<0.05).The scores of vertigo,headache,nausea and vomiting and balance disorder in the two groups after treatment were lower than those before treatment.The scores of vertigo,headache,nausea and vomiting and balance disorder in the observation group after treatment were lower than those in the control group(P<O.05).After treatment,the DARS score and DHI score decreased more significantly than those in the control group(P<0.05).Conclusion Wuzhuyu Decoction combined with betastine can improve the clinical symptoms of vertigo,reduce TCM syndrome score,improve cerebral blood flow,and greatly improve the therapeutic effect and quality of life,which is worthy of further study.
